AbstracteHealth systems have been created in most developing countries to facilitate the functioning of healthcare operations and as such various healthcare applications are existing as fragmented silo systems. Such autonomous standalone systems do not communicate through a network thereby making it a challenge to share healthcare data. Therefore, to re‐engineer health information systems (HISs) to develop interoperable adaptive enterprise architecture (EA) systems for healthcare, the study aimed to discover and capture healthcare interoperability functional imperatives through understanding the expectations of healthcare practitioners and patients on post‐development of an interoperable HIS. Therefore, the study's aim was to determine the HISs interoperability perceived functional expectations by both patients and practitioners in fulfilling their healthcare receipt and provision needs in an integrated healthcare environment, respectively. The Open Group Architecture Framework (TOGAF) was used as the main study framework to guide the classification of the expectations from the HIS by patients and practitioners deriving the needs from the four domains which are, business architecture (BA), application architecture (AA), data architecture (DA), and technical architecture. The study used interviews and questionnaire surveys to collect qualitative and quantitative data respectively. The study used purposive sampling to select interview participants. A total of 19 interviews were conducted with healthcare practitioners. Questionnaires were collected from 71 healthcare practitioners and 143 patients and analyzed quantitatively respectively to understand the most significant needs anticipated in an interoperable HIS. The research targeted age groups of at least 20 years and above. The study discovered that patients and practitioners expect the interoperable healthcare environment to support the acquisition of disease knowledge through healthcare surveillance synergies; create healthcare awareness through coordinated digital interactions; augmentation of healthcare intelligence for patient‐care through the healthcare knowledgebase; allow treatment collaboration by various healthcare practitioners in the healthcare ecosystem and mostly achieving a guaranteed healthcare system security and assurance environment.

The Design and Construction of a 12-Channel Electrocardiogram Device Developed on an ADS1293 Chip Platform

An accurate and compact electrocardiogram (ECG) device will greatly assist doctors in diagnosing heart diseases. It will also help to address the increasing number of deaths caused by heart disease. Accordingly, the goal of the project is to design and construct an easy-to-use compact 12-lead electrocardiogram device that communicates with a computer to create a system that can continuously monitor heart rate and which can be connected to allied medical systems. The design is based on an ECG receiver circuit utilizing an IC ADS1293 and an Arduino Nano. The ADS1293 has built-in input Electromagnetic Interference (EMI) filters, quantizers, and digital filters, which help in reducing the size of the device. The software has been created using the C# programming language, with Windows Presentation Foundation (WPF), aiding the collection of the ECG signals from the receiving circuit via the computer port. An ECG Multiparameter Simulator has been used to calibrate the ECG device. Finally, a plan has been developed to connect the arrangement to health systems according to HL7 FHIR (Health Level Seven Fast Healthcare Interoperability Resources) through Representational State Transfer Application Programming Interface (Rest API). The ECG device, completed at the cost of U$169 excluding labor, allows for the signal of 12 leads of ECG signal to be obtained from 10 electrodes mounted on the body. The processed ECG data was written to a JSON file with a maximum recording time of up to three days, managed by a Structured Query Language Server (SQL) Server database. The software retrieves patient data from electrical medical records in accordance with HL7 FHIR standards. A compact and easy-to-use ECG device was successfully designed to record ECG signals. An in-house developed software was also completed to display and store the ECG signals.

Cyber-physical systems (CPS) are recognized for their intelligence as they seamlessly interact with humans, enhancing the physical world through computation, communication, and control. Over the last few years, the evolution of CPS, including IoT components, has significantly impacted many facets of people's lifestyles. It has been immersed in a wide range of services and applications in various areas, including manufacturing, healthcare, and energy. However, the interrelationship between the cyber and physical worlds gives rise to a multitude of research problems and challenges. In the healthcare field, for instance, CPS introduces complexities related to interoperability, privacy, data security, and real-time data processing with critical implications for the reliability and safety of medical processes. To address these challenges and harness the full potential of CPS in healthcare, this paper, through a comprehensive literature review, aims to discuss cutting-edge CPS technologies and solutions that hold promise for healthcare applications. To this end, we propose a comprehensive architectural model that can serve as a benchmark for implementing CPS in healthcare applications. This model thoroughly details how services, components, and technologies can be integrated to transform massive raw data collected from the physical world into valuable information for an enhanced decision-making process. Finally, a use case on healthcare CPS is presented, outlining its characteristics, the role that different technologies have played in its development, and the major challenges in implementing such systems successfully. This study provides a cohesive understanding of the role CPS can play in empowering healthcare while offering insights into the challenges, and future research trends.INDEX TERMS Cyber-physical systems, Healthcare, Real-Time processing, IoT.
