Eccrine Porocarcinoma: Clinical and Pathological Report of Eight Cases

Abstract: Background: Eccrine porocarcinoma (EPC), a slow-growing carcinoma of the sweat gland, is a rare condition documented only in a small number of case series. Due to its rarity, guidelines and specific recommendations are not widely available. Accordingly, many dermatologists encounter difficulty in diagnosing and treating EPC. The aim of this study is to report the clinical and pathological features of EPC in order to contribute to the body of information currently available on the subject.

“…These lesions are composed of large, basaloid and atypical neoplastic cells with hyperchromatic nuclei and some mitotic figures. Duct-like structures with eosinophilic cuticular borders are constant findings [15]. On the other hand, immunohistochemical features like positivity for carcinoembryonic antigen or epithelial membrane antigen help to confirm the diagnosis of difficult cases where histopathological findings are not conclusive [4].…”

“…Gross total surgical excision with safety margin is the main stone of management of EPC with cure rate of 70-80% [9,15]. Lymph nodes dissection is to be considered in case of regional lymph node involvement [10].…”

“…They named it epidermotropic eccrine carcinoma. Since then, in literature there have been published numerous case reports [4][5][6][7][8][9][10][11]. Due to the rarity of this oncological entity, accurate data on its incidence, etiopathogenesis, and biological behaviour are quite poor.…”

“…The original study series have demonstrated local recurrences in 4.1-35% [4-7, 10, 11] and metastases in 4.1-50% of cases [4][5][6][7][8][9][10]. Despite marked percentage variance, these data suggest that EP is likely an aggressive and prognostically unfavorable type of cutaneous neoplasia.…”
