EBV-induced polymorphic lymphoproliferative disorder of the iris after heart transplantation

Abstract: PTLD is a well-known, EBV-induced entity that rarely affects the eye. EBV is principally detectable in specimens of iris PTLD. If conservative, antiviral treatment fails, the iris lesions can be treated by local radiation therapy with very good success. In the near future, patients with PTLD of the eye may benefit from immunologic treatment with ex vivo generation of virus-specific T-lymphocytes.

“…Although PTLD is one of the most common conditions occurring after solid organ transplantation, ocular PTLD is very rare with only 19 cases reported in the literature (Table I). Bilateral involvement occurred in around half of the cases [1,4,[9][10][11][12][13]15]. The time interval from transplantation to ocular PTLD varied from 3 months to 10 years.…”

“…Although the treatment of the above mentioned malignancies would follow own protocols, posttransplantation lymphoproliferative disease, caused in children mostly by EpsteinBarr virus, has recently been shown in a case report to be effectively treated by the use of ex vivo-generated Epstein-Barr virus-specific cytotoxic T lymphocytes. 20 …”

“…Recently, our group published the case of a young boy who had developed severe anterior uveitis with iris nodules and secondary glaucoma. 47 The iris biopsy disclosed PTLD with the presence of EBV and massively elevated viral load. The elimination of EBV finally succeeded with the use of ex vivogenerated EBV-specific cytotoxic T-lymphocytes.…”
