EBUS-TBNA Can Distinguish Sarcoid-Like Side Effect of Nivolumab Treatment from Tumor Progression in Non-Small Cell Lung Cancer

Abstract: With the expansion of immunotherapy in the treatment of lung cancer, clinicians have to face new clinical pictures and adapt their practice. We report the case of a 69-year-old man diagnosed with non-small cell lung cancer using endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) and treated with nivolumab as second-line therapy. After 8 injections of nivolumab, a new CT and PET scan revealed massive growth and increase in metabolism of hilar and mediastinal lymph nodes, whereas the si… Show more

“…Concerning clinical presentation, the time to onset was 12 weeks, and the patients were most often asymptomatic or developed dyspnoea, cough or asthenia [112][113][114][115][116][117][118][119][120][121][122][123][124][125][126][127][128]. Thoracic CT scans revealed stage I-II sarcoidosis-like presentation in 14 of the 19 patients (73.6%) (figure 2c).…”

“…Thoracic CT scans revealed stage I-II sarcoidosis-like presentation in 14 of the 19 patients (73.6%) (figure 2c). However, atypical lesions were observed, such as ground-glass opacities (n=5) (figure 2d) and low abundance pleural effusion (n=2) [112][113][114][115][116][117][118][119][120][121][122][123][124][125][126][127][128]. Extrathoracic involvement was observed in 10 (52.6%) patients, of whom nine had melanoma, with lesions affecting skin (n=5), spleen (n=3), eyes and parotid gland (n=1) and brain (n=1) [114][115][116][117][118][120][121][122][123][124], in line with sarcoidosis presentation [133].…”

“…For lung cancer, there have been reports of skin sarcoidosis during the treatment with nivolumab alone or in combination with ipilimumab [6, 7]. Moreover, granulomatous lesions have been observed in the mediastinal lymph node with the use of nivolumab alone [8]. However, the present case may be the first case to demonstrate granuloma development in the lung.…”

“…The granulomas are typically composed of epithelioid cells, giant cells, CD4+ T‐lymphocytes at the centre, and CD8+ T‐lymphocytes and B‐lymphocytes at the periphery, surrounded by a fibrotic rim in sarcoidosis . In sarcoid‐like reaction, this rim may be lacking . Obviously, sarcoid‐like reaction produces a similar pattern on BAL compared to sarcoidosis.…”
