Background: Coronavirus disease 19 (COVID-19) has affected many aspects of the lives of medical professionals. Postgraduate training has also been affected and mitigation plans are still ongoing. Objective: To understand the perspectives of trainees in obstetrics and gynaecology (ObGyn) during the pandemic. Materials and Methods: A cross-sectional exploratory survey conducted electronically from 20th of April 2020 to 1st July 2020. Main outcomes measures: The original questionnaire comprised of 40 questions and a free-text option. The free-text questions covered five main domains: effect of the pandemic on training, worries about training, acquisition of skills during the pandemic, training period and extensions and responsibilities outside training during the pandemic. The responses to these questions in the survey were analysed using pragmatic thematic analysis. Results: Trainees felt there was lack of training as well as training opportunities. Some took the pandemic as an opportunity to gain new skills. Trainees were also worried about time in training and uncertainty about extensions. Lastly, many had concerns pertaining to patient care, an inability to contribute to departmental organisation, and dissatisfaction with the implemented policies. Conclusion: The difficulties in Obstetrics and gynaecology training due to the pandemic need to be mitigated. When planning for reshaping the training programmes to accommodate for the discrepancies caused, trainers need to consider the perspectives of trainees and actively involve them in the decision making, designing and executing future plans. What is new? Efforts are currently underway to address the training time lost during the pandemic in Europe. Recognising the paramount importance of providing exceptional care for women and children across the continent, it becomes imperative to consider the valuable perspectives and insights offered by those who represent the future generation of specialists in the field.

Background & Objectives: Obstetrics and Gynaecology (O&G) training programmes that traditionally relied on the hands-on apprenticeship-training model, became crippled with the global response to the COVID-19 pandemic. Methods: Web-based anonymised survey was circulated to trainee members of the European Society for Gynaecological Endoscopy (ESGE) over 8-weeks period commencing June 2021. Results: 213 trainees from 20 countries responded. Trainees from medium Human Development Index (HDI) countries were less represented. 78% (166/213) were in approved training programmes and 81% (174/213) had access to personal PPE. The vaccine uptake was 87% (185/213). 39% (89/213) and 55% (118/213) experienced negative impact on their physical and mental wellbeing with 36% (76/213) COVID-19 related absence. 15% (32/213) were redeployed to areas outside O&G. 25% (53 /213) had negative impact on their obstetric experience compared to 54% (114/213) reported lower gynaecology surgical exposure and 43% (91/213) failed to meet their gynaecology surgical competencies during the pandemic. 64% (137/213) perceived simulation training as an alternative training tool. Conclusion: In the post-pandemic recovery phase, gynaecological societies and national institutes across Europe continue to develop training curricula implementing virtual and hybrid training modules. The aim is to develop a robust blueprint to safeguard the gynaecological surgical training in the future. What is new? The ongoing impact on the training in the post pandemic era remains to be evaluated. Our pan Europe survey highlights areas that remain affected from trainees’ perspective and assesses differences in the healthcare systems across continent. We then discuss the novel initiatives taken to overcome training gaps.

ZusammenfassungEine geburtshilfliche Simulation gibt den Teilnehmenden die Möglichkeit, realitätsnah klinische Szenarien durchzuspielen und sowohl technische als auch „non technical skills“ im Team zu trainieren. Die anschließende Nachbesprechung evaluiert die Simulation. Mit zunehmender Verunsicherung im Ausführen geburtshilflicher Manöver durch fehlende praktische Erfahrung und Angst vor Verantwortung können Aufbau und Erhalt von Simulationstrainings auch einen attraktiven neuen Weg darstellen, das Interesse für die Geburtshilfe zu stärken und zu fördern. Bisher zeigten sich positive Ergebnisse in der Zufriedenheit der Teilnehmenden, in der Gewinnung von Sicherheit beim Durchführen der Manöver, in der Verbesserung der interdisziplinären Teamarbeit und im klinischen Outcome. Die Durchführung der Simulationen ist kosten- und zeitintensiv; Teamwork und Kommunikation lassen sich nur im interdisziplinären und interprofessionellen Setting (Hebammen, Geburtshelfer/innen, Anästhesisten/innen) trainieren. Grundvoraussetzungen der Simulation sind Unterstützung durch die Klinikleitung, Motivation der Teilnehmenden und in Debriefingverfahren geschulte Trainer.
