Urinary tract infection (UTI) is a widespread disease, and thus, the most common samples tested in diagnostic microbiology laboratories are urine samples. The "gold standard" for diagnosis is still bacterial culture, but a large proportion of samples are negative. Unnecessary culture can be reduced by an effective screening test. We evaluated the performance of a new urine cytometer, the Sysmex UF-1000i (Dasit), on 703 urine samples submitted to our laboratory for culture. We compared bacteria and leukocyte (WBC) counts performed with the Sysmex UF-1000i to CFU-per-milliliter quantification on CPS agar to assess the best cutoff values. Different cutoff values of bacteria/ml and WBC/ml were compared to give the best discrimination. On the basis of the results obtained in this study, we suggest that when the Sysmex UF-1000i analyzer is used as a screening test for UTI the cutoff values should be 65 bacteria/ml and 100 WBC/ml. Diagnostic performance in terms of sensitivity (98.2%), specificity (62.1%), negative predictive value (98.7%), positive predictive value (53.7%), and diagnostic accuracy (73.3%) were satisfactory. Screening with the Sysmex UF-1000i is acceptable for routine use. In our laboratory, we have reduced the number of bacterial cultures by 43%, speeded up their reporting, and decreased the inappropriate use of antibiotics.The urinary tract is the most common site of infection, and urine culture is the standard diagnostic test for urinary tract infection (UTI). The growing need to enhance the performance of urine culture combined with the need to free up resources by rejecting negative samples quickly and economically has drawn attention to solutions that can be used as screening tests to reduce the number of unnecessary culture tests. In this study, we sought to evaluate and optimize the performance of the Sysmex UF-1000i (Dasit) on 703 urine samples submitted at the same time to bacterial culture analysis. MATERIALS AND METHODSA comparison between the Sysmex UF-1000i urinary cytometer and urine culture was conducted on 703 samples, 128 of which (18.2%) were from hospitalized patients and 575 (81.8%) from outpatients. The urine samples were collected in sterile containers using the technique of midstream capture and were processed within 2 h of collection (5, 10). Each sample was divided into two sterile tubes with a vacuum system (Vacutainer; Becton-Dickinson); one tube was used for urine culture and the other for analysis by the Sysmex UF-1000i.The urine culture was performed by sowing 10 l of the urine sample on chromogenic agar plates (CPS agar; bioMérieux) with calibrated loops. CPS agar is a specific medium for isolation of bacteria in urinary samples. Epididymoorchitis is predominantly an acute inflammatory process. However, if left untreated, chronic inflammation may develop with the sequelae of abscess formation, testicular infarction, testicular atrophy and infertility. 1 In men aged >3 5 years, the majority of cases are due to infection with common urinary pathogens and may involve other risk factors such as bladder outlet obstruction and urogenital malformations. In men aged ≤ 35 years who are sexually active, acute epididymo-orchitis may be associated with sexually transmitted organisms and infection of the consort. 2,3 Infection by Chlamydia trachomatis remains the most common bacterial sexually transmitted disease (STD) in the UK and studies have shown that acute epididymoorchitis in young males is frequently associated with C. trachomatis infection. 4,5 The increasing incidence of acute epididymo-orchitis in this younger age group is occurring alongside a nation-wide increase in the transmission rates of C. trachomatis infection and is part of a wider public health crisis of spiralling STD rates in the UK population. 6,7 UROLOGY

Сеченова, Москва, Россия Проблема разработки новых эффективных методов лечения пациентов с хроническим бактериальным везикулитом (ХБВ) является весьма актуальной. Цель -дать научное обоснование применения синусоидальных модулированных токов (СМТ), магнитного поля и лазерного излучения в комплексной терапии пациентов с ХБВ. Пациенты и методы. Проведено обследование и лечение 121 пациента с ХБВ в латентной фазе активности воспалительного процесса. Пациенты были рандомизированы на 3 группы: в 1-ю (основную) группу вошли 40 человек, получавшие на фоне базовой лекарственной терапии надсосудистое лазерное облучение на область кубитальной вены по контактной стабильной методике с примене-нием через 2-3 ч СМТ по лонно-крестцовой методике, во 2-ю (основную) группу был включен 41 пациент, которым на фоне базовой лекарственной терапии проводилась лазеротерапия по аналогичной методике с применением через 2-3 ч полостной магнитотерапии с помощью ректального индуктора, 3-ю (контрольную) группу составили 40 больных, получав-ших только лекарственную терапию, включавшую антибактериальные и противовоспалительные средства. Результаты. Установлено, что наиболее выраженная положительная динамика клинической симптоматики по данным суммарного по-казателя по шкале оценки симптомов хронического простатита NIH-CPSI (National Institute of Health Chronic Prostatitis Symptom Index) и улучшение качества жизни по шкале QOL (Quality of Life Scale) отмечены во 2-й группе. В основных группах происходило более выраженное снижение активности воспалительного процесса в семенных пузырьках, восста-новление их структурно-анатомических изменений по данным трансректального ультразвукового исследования, улучше-ние метаболизма тестостерона и нормализация показателей спермограммы по сравнению с группой контроля. При ис-следовании спермограмм тенденция к увеличению активно подвижных форм сперматозоидов была выявлена только у больных 2-й группы. В результате комплексной терапии у пациентов основных групп отмечено снижение концентрации глобулина, связывающего половые гормоны, и увеличение индекса свободных андрогенов. Заключение. Таким образом, применение преформированных физических факторов у пациентов с ХБВ повышает эффективность лекарственной тера-пии и снижает длительность и частоту возникновения рецидивов заболевания.
