Early-Stage Young Breast Cancer Patients: Impact of Local Treatment on Survival

Abstract: In this large population-based cohort of early-stage young breast cancer patients, 10-year OS was not impaired after BCT compared with mastectomy. Patients with 1 to 3 positive lymph nodes had better prognosis after BCT than after mastectomy.

“…Six randomized controlled trials comparing mastectomy and BCT all included a low proportion of young breast cancer patients (12-23%) [44], making it hard to draw any conclusion for this subgroup. Few cohort studies restricted to young breast cancer patients have been published [2,[45][46][47][48][49], and they are limited by the inclusion of a mixture of different TNM stages, by the variety of adjuvant systemic treatments offered, and by a median follow-up time of less than 10 years. In contrast, the present study is based on an unbiased cohort (comprising almost all lymph node negative patients below 41 years treated in Denmark in the 1989-1998 period) with complete registration of incident breast cancer and complete follow-up data on LRR and mortality within 20 years.…”

“…In recent years, several large retrospective analyses of contemporary data have suggested the stage‐adjusted superiority of BCS over mastectomy in early breast cancer in terms of breast cancer‐specific survival and overall survival. Other publications have focused on young women or triple‐negative breast cancer, with the results indicating that BCS is at least as good as mastectomy in terms of survival outcomes. This interesting finding has led some authors to question whether mastectomy without RT should be offered at all as a treatment alternative for early breast cancer, or whether it is time to abandon the ‘mastectomy myth’.…”
