Early Results of the Bolton Treovance Endograft in the Treatment of Abdominal Aortic Aneurysms

Abstract: Treovance shows accurate deployment even through angulated and tortuous iliac vessels and presents satisfactory conformability in highly angulated necks with acceptable clinical results. Future development to lower the profile and increase the flexibility of its delivery system will enhance its applicability in cases of narrow access vessels.

“…Early clinical studies with the Treovance abdominal stentgraft system in AAA patients (including hostile anatomies) demonstrated feasibility and safety. [21][22][23][24] However, these investigations involved relatively small cohorts. The more robust RATIONALE registry results show favorable 1-year safety and clinical performance of the Treovance stent-graft in a real-world setting, with 54% of patients having a ⩾5mm reduction in aneurysm sac size at 1 year compared with 32% in OVATION, 26 36% in GREAT, 27 and 41.3% in ENGAGE.…”

“…The stent-graft performed favorably in the ADVANCE 21 clinical trial as well as single-center and dual-center cohorts. [22][23][24] The objective of this global registry was to evaluate safety and performance of the Treovance stent-graft in patients with infrarenal AAAs.…”

“…The length of the iliac limbs also extends from 80 to 160 mm. 5 These characteristics render this endograft more suitable for patients such as those observed in the HNA group with short common iliac arteries. Notably, the unique Lock-stent mechanism of Treovance enhances stability in the conjugated iliac limbs, counteracting the high migration forces associated with long and tortuous endograft configurations.…”

“…1 The extra infrarenal barbs remain concealed in the valley of the first covered stent and are designed to hook in place in cases of significant infrarenal neck angulation, providing extra fixation. 2,3 In the December 2018 issue of the JEVT, the RATIONALE registry 4 reports 1-year results from 202 patients treated with the Treovance platform. The authors show favorable results, with both technical and clinical success in 96% of patients, 1-year freedom from reintervention of 95.6%, and 1-year freedom from type I/III endoleak of 95.5%.…”

“…Our experience using this stent-graft in the "realworld setting" of patients with hostile neck anatomy showed encouraging short-term results. 2 Nonetheless, the structural Journal of Endovascular Therapy 25 (6) characteristics of Treovance incorporate the advanced technology of double supra-and infrarenal fixation, a flexible delivery system with satisfactory conformability in angled anatomy, and an easy-to-use deployment device. 1 All these theoretical characteristics and existing clinical data suggest the hypothesis that Treovance will prove to be an equally safe and efficient stent-graft for challenging cases in the long term.…”
