Early primary abdominal pregnancy: Diagnosis and management. A case report

Abstract: Highlights The clinical presentaion of early abdominal pregnancy is similar to that of tubal ectopic pregnancy in the majority of cases. Surgical laparoscopy must be the first choice in management of early abdominal pregnancy. Medical treatment should be reserved when a surgical intervention is deemed to be potentially very hemorrhagic.

“…Monitoring serum beta-human chorionic gonadotropin levels is a useful marker for clinical suspicion of EP; however, it is not sufficient by itself to make the diagnosis of EP. In many cases, the diagnosis of AEP is not confirmed until a laparotomy or laparoscopy is performed due to suspicion of a tubal EP [3, 10, 14, 19, 21].…”

“…Diagnostic imaging methods include MRI and US. The latter can reach a sensitivity of 99% if used properly [13, 19, 21]. When observed during a US, an empty uterus with gestational sac separated from the uterus, adnexa, and ovaries should raise suspicion of AEP.…”

“…Meanwhile, secondary implantation occurs after tubal rupture, tubal abortion, or uterine rupture. Secondary implantation is the most frequently occurring process in abdominal pregnancies [3,6]. In some cases, patients experienced a history of abdominal pain during the first trimester, and the uterus and both fallopian tubes were found intact.…”

“…Conceptual implantation in advanced abdominal pregnancy can take place in the omentum, ligamentum latum, abdominal organs, or blood vessels located in the abdomen and pelvis. The estimated occurrence of advanced abdominal pregnancy with live birth is about 1:10,000 to 1:30,000 from all pregnancies and 1% from ectopic pregnancy [1][2][3]. Advanced abdominal pregnancy occurs as a result of fertilization taking place in the abdominal cavity, tubal rupture, and tubal abortion.…”
