Early Predictors of Ten-Year Course in First-Episode Psychosis

Abstract: First-episode psychosis patients should be followed carefully after the start of treatment. If symptoms do not remit within three months with adequate treatment, there is a considerable risk of a poor long-term outcome, particularly for patients with a deterioration in premorbid social functioning, a DUP of at least half a year, and a diagnosis within the core schizophrenia spectrum.

“…The model uses a cohort of 10 000 hypothetical patients. Although they differ with respect to DUP at treatment entry, patients are assumed to have comparable sociodemographic and clinical characteristics associated with long‐term outcomes such as age, premorbid social functioning, and symptom profile (eg, severity of negative symptoms; Friis et al, ).…”

“…We use the term "Time in Uncontrolled Psychosis" to refer to the sum of DUP and time in psychosis accrued due to inadequate symptom control following treatment entry. The TUP concept is supported by evidence that time in psychosis accrued immediately after entering treatment is at least as consequential for long-term outcomes as the DUP (Friis et al, 2016), suggesting a critical role for overall time in psychosis at this early stage.…”

“…The effects of DUP and other variables on outcome were examined by Friis et al in a study that was a part of the Scandinavian TIPS project mentioned above (see Hegelstad et al and Weibell et al). Four variables provided significant, additive predictors of longer time in psychosis during the 10‐year follow‐up: DUP of ≥26 weeks, diagnosis of core schizophrenia spectrum disorder, no remission within the first 3 months of illness and deterioration in premorbid social functioning.…”

“…Pasientene oppga at de hadde brukt antipsykotika i 65,7 % av oppfølgingstiden på ti år (6,16,17). Det er altså ikke slik at en person med en psykoselidelse trenger medikamenter for alltid, og det er mulig å få behandling uten antipsykotika.…”
