Early predictors of oxaliplatin-induced cumulative neuropathy in colorectal cancer patients

Abstract: High-grade OXA neurotoxicity can be predicted by clinical and neurophysiological information obtained at mid-treatment. Neurological assessment of acute neuropathy symptoms and radial and dorsal sural nerves NCS should be carefully monitored to predict and hopefully prevent the induction of severe OXAIPN.

“…Studies show that the neurotoxic side effects are greatly affected by cumulative doses and are predominantly found in men [14] [16] [18]. These side effects may be permanent and should be weighed against the increased chance of survival that oxaliplatin gives [3] [15] [32].…”

“…Permanent damage may occur without acute side effects in the initial stage [13]. Studies show that the neurotoxic side effects greatly affect the duration of chemotherapy; therefore, deciding whether the dosage of oxaliplatin should be reduced or treatment stopped for fear of debilitating and prolonged neurotoxicity is an important issue [14]- [18]. Although a number of interventions for chemotherapy-induced neurotoxicity have been tested, none of them can be recommended for clinical use due to lack of evidence [19]- [21].…”

“…This would potentially abate the need to terminate chemotherapy treatment by permitting early identification of patients at risk of high-grade neuropathy allowing prompt chemotherapy dose reduction before nerve damage occurs. 78 Work is currently ongoing to identify and investigate novel biomarkers for CIPN.…”

“…Oxaliplatin is one of these drugs and it is frequently used today, usually combined with 5-fluorouracil (5-FU) and folinic acid (leucovorin) (1)(2). Survival among patients with CRC has been shown to improve when oxaliplatin is added to the treatment, even when the patient is in later stages of the disease when diagnosed (3).…”

“…It is also known, in comparison with other combinations, that patients receiving a combination that includes oxaliplatin have their symptoms for a longer time than patients receiving other combinations (5). Oxaliplatin has recently been introduced in the treatment of other forms of cancer in the gastro-intestinal tract (GI-tract), such as oesophagus-, gastric-and pancreatic cancer (2,(10)(11).…”
