Early postoperative transtibial articular fistula formation after anterior cruciate ligament reconstruction: a review of three cases

Abstract: All wounds healed without complications. There was no recurrence of drainage or cyst formation. At 2 years follow-up the knee function was normal and was not affected by the complication in any of the patients. Early postoperative transtibial fistulae after ACL reconstruction are rare complications that clinically present either as anterior tibial cysts or persistent wound drainage. Surgical treatment is required, and some delay in the rehabilitation routine is required, but the final outcome is not affected.

“…The publication dates of the included studies ranged from 1999 to 2019. Using the adjusted Oxford Center For Evidence‐Based Medicine criteria [74, 75] for the level of evidence we found that 1 study was Level I [11], 1 Level II [25], 1 Level III [57] and 34 studies were level IV [1, 3, 10, 12–17, 19–21, 23, 29, 33, 36, 42, 43, 46, 49, 51, 53, 54, 56, 58, 59, 62, 64–66, 69, 71, 72] case series or case reports.…”

“…Synovial fistula (SF) of joint is a rare pathologic condition that is formed through a defect within the joint capsule. There have been several reports of SF around the knee, shoulder, and wrist joints, which developed in the wake of intra-articular surgeries such as arthroscopic procedures [1][2][3][4][5][6][7]. Regarding its mechanism, SF operates as a check-valve and drains the synovial fluid externally into the extracapsular space.…”

“…Case Reports in Orthopedics Synovial fistula is an uncommon complication (0.61%-0.011% incidence) [2][3][4]. It usually appears through arthroscopy portals, either chronic [5,8] or acute [9]. This complication could present as a fistula if opened to skin [4,9,10] or as a cyst if skin is preserved [5-7, 9, 11-13].…”
