Early Management of Infants With Robin Sequence: An International Survey and Algorithm

Abstract: There is no widely accepted protocol for management of infants with Robin sequence (RS) who present with airway obstruction and feeding impairment. The purposes of this study were to evaluate diagnostic and treatment preferences of clinicians from the United States and non-US countries and to use these data to propose an algorithm for early management of infants with RS.Materials and Methods: A cross-sectional study was implemented using a survey, which was distributed to craniofacial surgeons and non-surgeon … Show more

“…These temporizing options are appropriate when mild anatomic differences can be compensated through catch-up growth. Tongue-lip adhesion is a relatively safe procedure that tethers the anterior tongue to the lower lip; however, it has fallen out of favor at some centers due to inconsistent outcomes (Resnick et al, 2019). Tracheostomy and gastrostomy tube insertion are the gold standards for definitively bypassing both supraglottic and subglottic obstruction, but their associated morbidity restricts their routine use.…”

“…Mandibular distraction osteogenesis has been shown to significantly improve or cure OA (Resnick et al, 2019), simplify laryngoscopy and intubation (Heffernan et al, 2019), and improve feeding (Ghoul et al, 2018) in infants with RS. Direct laryngoscopy and intubation at the time of the initial operation to perform the mandibular osteotomies and place the distraction devices is often extremely challenging due to unfavorable facial and airway anatomy.…”

“…A recent international survey showed wide variation of management of these infants. 47 A 2016 consensus report recommended endoscopic airway evaluation to confirm glossoptosis and diagnose concomitant airway abnormalities. If concomitant abnormalities cannot be corrected, then tracheostomy is recommended.…”
