Grover’s disease in a patient with atopic dermatitis - a case report

Introduction. Grover?s disease is characterized by pruriginous polymorphic rash with a variable course and duration. Although the etiology is still unknown, the disease is often associated with other dermatoses, malignant diseases, use of certain medications, as well as immunosuppression. Case Report. We report a case of a 70-year-old male patient who was referred for examination to the Clinic of Dermatovenereology Diseases, Clinical Center of Vojvodina, due to a rash that lasted for nine months. The first lesions on the skin appeared around the nipples as exudative eczematous plaques. A few months later, identical lesions appeared on the lower legs. During treatment with systemic antihistamines and topical corticosteroids, there were episodes of transient improvements and re-exacerbations. In the meantime, erythematous brownish, round and oval papules appeared on the abdomen and the back, accompanied by intense itch. Laboratory findings revealed eosinophilia and elevated serum immunoglobulin E levels. A skin biopsy of the back lesion was performed and the histopathological examination confirmed the diagnosis of Grover?s disease. After the systemic treatment using corticosteroids and antihistamines, with gradual dose reduction and application of topical corticosteroids and emollients, complete regression of the skin lesions was achieved. Conclusion. Since the clinical manifestations of the disease may be nonspecific and discrete, dermatopathological analysis is of crucial importance in making the correct diagnosis. In patients with atopy, the treatment with systemic corticosteroids, antihistamines and topical agents may lead to regression of skin lesions with a significant improvement in the quality of life.

Η παροδική ακανθολυτική δερματοπάθεια περιγράφηκε για πρώτη φορά από το δερματολόγο RW Grover το 1970 στη Νέα Υόρκη. Η νόσος Grover παρουσιάζει μεγάλη ποικιλομορφία τόσο στην κλινική εικόνα, όσο και στα υποκειμενικά συμπτώματα των ασθενών. Η ιστολογική έκφραση της νόσου ακολουθεί αυτή την ποικιλομορφία εμφανίζοντας εικόνα από αμιγώς ακανθολυτική έως εικόνα τύπου αλλεργικής εξ επαφής δερματίτιδας.Η προσπάθειά μας είχε στόχο τη σε βάθος μελέτη των διαφόρων μορφών της ιστολογικής εικόνας της νόσου και της σύνδεσής τους με πιθανούς παθογενετικούς μηχανισμούς. Έτσι συγκεντρώσαμε 110 περιστατικά νόσου Grover τα οποία χωρίσαμε σε 3 ομάδες βάσει της ιστολογικής τους εικόνας:Α) ΔΕΡΜΑΤΙΤΙΔΑ ΜΕ ΑΚΑΝΘΟΛΥΣΗ Β) ΚΥΡΙΩΣ ΑΚΑΝΘΟΛΥΣΗ με ελάχιστη έως καθόλου δερματίτιδα (εικόνα τύπου πέμφιγας-κοινής και φυλλώδους, τύπου Darier και τύπου πέμφιγας Hailey-Hailey Γ) ΜΕΙΚΤΟΣ ΤΥΠΟΣ όπου η ιστολογική εικόνα της πρώτης ομάδας και της δεύτερης ομάδας καταλάμβαναν στο ιστολογικό παρασκεύασμα το ίδιο ποσοστό περίπου.Ο μέσος όρος ηλικίας εμφάνισης της νόσου στη μελέτη μας ήταν τα 57, 2 έτη,και η εμφάνιση της νόσου ήταν περίπου ισομοιρασμένη μεταξύ των δύο φύλων (56 άνδρες/54 γυναίκες). Ο πιο συχνός ιστολογικός τύπος ήταν αυτός της δερματίτιδας με μικρού βαθμού ακανθόλυση, ενώ από τη δεύτερη ομάδα η πιό συχνή εικόνα ήταν αυτή της φυλλώδους πέμφιγας.Σε όλα σχεδόν τα περιστατικά παρατηρήθηκε κάποιου βαθμού διήθηση από ηωσινόφιλα στο χόριο σε ποσοστό μεγαλύτερο από αυτό που έχει αναφερθεί μέχρι τώρα.Παρόλα αυτά δεν έχει αναφερθεί μέχρι σήμερα το μεγάλο ποσοστό της εισόδου των ηωσινοφίλων στην επιδερμίδα, καθώς και το ποσοστό των ηωσινοφίλων σε επαφή με νεύρα.Παρατηρήθηκε επίσης αυξημένη παρουσία πλασματοκυττάρων στις ιστολογικές τομές με πιο συχνή εμφάνιση στη δεύτερη ομάδα και βαθμιαία μείωση της παρουσίας τους στις άλλες δύο ομάδες. Πιστεύουμε ότι η στατιστικά σημαντική παρουσία τους (p<0.0001) υποδηλώνει πιθανά ένα ξεχωριστό παθογενετικό μηχανισμό σε αυτή την ομάδα.Παρατηρήσαμε κάποιο βαθμό ινοβλαστικής αντίδρασης στο χόριο σχεδόν στα μισά περιστατικά (45,5%), γεγονός που θεωρούμε ότι οφείλεται στην αυξημένη παρουσία μαστοκυττάρων.Ένα ακόμα εύρημα που αναφέρεται για πρώτη φορά είναι η αύξηση του αριθμού τωνLangerhans κυττάρων στη νόσο του Grover. Η παρουσία των Langerhans κυττάρων φάνηκε να σχετίζεται με στατιστική σημαντικότητα με την ένταση της φλεγμονής.Εξετάσαμε την ύπαρξη βασεόφιλων στη νόσο του Grover και βρήκαμε θετική συσχέτιση της παρουσίας ηωσινόφιλων στο χόριο με τη διήθηση του χορίου από βασεόφιλα καθώς και θετική αναλογία μεταξύ ηωσινόφιλων στο χόριο και μαστοκυττάρων. Ενδιαφέρουσα παρατήρηση η οποία αναφέρεται και αυτή για πρώτη φορά είναι ότι η διήθηση του χορίου από μαστοκύτταρα και βασεόφιλα είναι ανάλογη από την αύξηση τωνLangerhans κυττάρων στην επιδερμίδα (p=0.001). Φαίνεται ότι για την αντίδραση του οργανισμού σε ένα ερέθισμα χρειάζεται ένας συνδιασμός διαφορετικών μηχανισμών οι οποίοι δρούν συμπληρωματικά.
