Total knee arthroplasty (TKA) was a well-established procedure that had shown excellent long-term results in terms of reduced pain and increased mobility. Pain was one of the most important outcome measures that contributed to patient dissatisfaction after TKA. After a computerized search of the Medline and Embase databases, we considered articles from January 1st, 1997 to October 31st, 2009 that underlined the impact on patient pain perception of either standard open total knee arthroplasty or minimally invasive total knee arthroplasty. We included articles that used the visual analog scale (VAS), Western Ontario and McMasters Universities Osteoarthritis Index (WOMAC), Knee Score, Hospital for Special Surgery Score (HSS), Oxford Knee Score (OKS) as postoperative pain indicators, and we included studies with a minimum follow-up period of two months. We excluded studies that monitored only functional postoperative knee activities. It was shown that TKA with the open technique was a better treatment for knees with a positive effect on pain and function than the minimally invasive technique.

Total knee replacement (TKR) is a surgical procedure with predictable outcomes. Rural areas have less specialized doctors and facilities and the people may not understand the various protocols associated with surgeries. This observational study intended to review the outcome of total knee replacements performed in a rural population and evaluate the postoperative clinical, functional and radiological results in the replaced knees and the preoperative and postoperative lower limb alignment correction achieved and the incidence of early complications. We selected 26 people (30 knees) from the outdoor clinic who gave their informed consent for the study and satisfied our criteria. They were evaluated using the knee society score (KSS), preoperatively and postoperatively at the 3 months follow up and the data was analysed using the statistical program for social sciences (SPSS). In our study we observed that the mean knee society score improved from 29 to 83.4 and the mean functional score improved from 35 to 83.5. The knee flexion range increased from 56.2 0 to 111 0 . Postoperatively all knees were aligned in valgus except for 3 knees. Complication rate was 6.6%. We found a significant increase in the knee society score after total knee replacement compared to the preoperative scores. We conclude that total knee replacement is a reliable and safe modality of treatment and can be performed in the rural population with results comparable to the other global studies.

Objective: To compare the postoperative blood loss in total knee arthroplasty with and without perioperative tourniquet release. Study Design: Randomized control trial Place & Duration of Study: Department of Orthopaedic, M. Islam Teaching Hospital Gujranwala 1st January 2020 to 31st December 2020. Methodology: Ninety patients of both genders undergoing total knee arthroplasty were included in this study. Patient’s demographics were recorded after written consent. Patients were equally divided into two groups. Group A (45 patients) received total knee arthroplasty with tourniquet while group B (45 patients) received total knee arthroplasty without tourniquet. Postoperative blood loss was measured by suction drain, levels of haemoglobin were examined and compare between two groups. Results: The mean age of the patients in group A was 57.76±2.4 years while in group B it was 58.16±7.8 years. 32 (71.11%) and 34 (75.56%) were males while 13 (28.89%) and 11 (24.44%) were females in group A and B. We observed patients at the three different points and found lower blood loss in patients who received total knee arthroplasty with tourniquet as compared to no tourniquet group (p-value <0.05). No significant difference was observed regarding hemoglobin at 2 hours and significant differences were observed at 24 and 48 hour postoperatively. Conclusion: Tourniquet is useful to decrease loss of blood in patients undergoing total knee arthroplasty. Keywords: Total knee arthroplasty (TKA), Tourniquet, Blood loss
