Background: To date, only few studies have reported on the clinical outcomes of immediate postextraction implant placement and immediate loading. Purpose:The purpose of this retrospective study was to report the results of immediately loading of four implants placed in fresh extraction sockets in the mandible after a follow-up of 24 months. Materials and Methods:Between January 2001 and January 2009, 50 patients (28 women and 22 men, average age 54 years) had 347 teeth extracted and a total of 200 dental implants placed in the mandible. The patients received a provisional fixed bridge the same day and a permanent one 3 months later. Clinical check-ups were performed after 1, 2, 3, 6, 12 and 24 months. Marginal bone measurements were made in intraoral radiographs taken one day after surgery and after one year. A questionnaire was used to evaluate self-perceived factors related to comfort, aesthetics and function.Results: All bridges were stable and no implant failures were recorded during the follow-up, giving a survival rate of 100%, at two years. The marginal bone loss amounted to 1.33 + 0.36 mm after one year and 1.48+ 0.39 mm after two years. Ten patients showed prosthetic complications with the provisional bridge, but all the definitive prostheses remained stable throughout the study period without any complications. The patients reported satisfaction with the treatment. Purpose: The aim of this retrospective study was to compare the survival rate of oxidized titanium implants and sandblasted large-grit acid etched implants in soft bone. Methods: 201 oxidized titanium implants were inserted in 84 patients between May 1999 and May 2004. 120 sandblasted large-grit acid etched implants were inserted in 74 patients between December 2000 and May 2004. The patients were followed-up 0~5 years in ITI group or 0~6 years in BRA group, respectively. The following information was collected from the patient records: age, gender, systemic disease, implant type, number, length and diameter of the implants, their location in the jaws, bone quantity, the number of failed implants, the causes of failure, and advanced surgery for bone augmentation. Results: In the oxidized titanium implants, 8 implants showed early failure, and 1 implant showed late failure, respectively. The cumulative survival rate was 95.48%. In the sandblasted large-grit acid etched implants, 1 implant showed late failure and cumulative survival rate was 99.10%. The cumulative survival rate and the survival rates in the case of the advanced procedure during the implant placement were not significantly different in both groups. Conclusions: Oxidized titanium implants and sandblasted large-grit acid etched implants can be used successfully in soft bone regardless of the surgical methods used during the implant placement. Все шире и смелее в амбулаторной практике при-меняются реконструктивно-восстановительные операции на альвеолярных отростках челюстей и верхнечелюстной пазухе. Лавинообразно растет количество устанавливаемых стомато-логами дентальных имплантатов, расширяются показания к дентальной имплантации [1].Современное состояние имплантологии характеризуется различным уровнем решения клинических задач по восста-новлению утраченных зубов с помощью имплантатов. Дис-куссионным остается вопрос о сроках начала протезирования после непосредственной имплантации [1,2].Главное условие достижения остеоинтеграции -тесный контакт между костным ложем и поверхностью имплантата непосредственно в момент оперативного вмешательства [3]. Поэтому на хирургическом этапе имплантации необходимо учитывать плотность костной ткани и первичную стабиль-ность имплантата.Для измерения плотности костной ткани шире всего применяется рентгеновская денситометрия, позволяющая объективно оценить состояние кости до имплантации; изме-рив плотность костной ткани в области прогнозируемой уста-новки ДИ, можно предупредить отдаленные осложнения, определить сроки проведения дальнейших этапов восстанов-ления жевательной функции и при благополучных показате-лях -сократить их.Стабильностью считают способность системы функцио-нировать, не изменяя собственную структуру, и находиться в равновесии.
