Early experience with feasibility of balloon sinus dilation in complicated pediatric acute frontal rhinosinusitis

Abstract: Background/Objective: Complicated acute rhinosinusitis in the pediatric population is an uncommon problem that may affect the orbit or brain and is life-threatening.This condition requires surgical intervention with endoscopic sinus surgery for source control, and prior studies have demonstrated the safety of balloon sinuplasty in chronic frontal sinusitis.Methods/Results: We present our approach with a balloon sinus dilation hybrid procedure to resolve four distinct types of complicated acute frontal sinusiti… Show more

“…Treating complicated pediatric frontal ARS with balloon sinuplasty has been previously described in one case report of a patient with an intracranial abscess and in a case series of four patients with the following complications: forehead swelling, frontal dural enhancement, intraorbital manifestation, and recurrent chronic sinusitis [ 13 , 14 ]. Multiple studies have established that balloon sinuplasty is safe and useful when examining short and long-term outcomes in pediatric chronic rhinosinusitis (CRS), though a consensus has not been reached for the indication of balloon dilation among children with CRS [ 15 23 ].…”

“…7 Recently, selective use of balloon frontal sinuplasty has been described to further mitigate the risks and challenges of endoscopic frontal surgery in an acutely inflamed pediatric patient. 12,13 Indications for addressing the frontal sinus in the surgical management of pediatric acute sinusitis with intracranial complications are not defined. Surgeons treating sinogenic intracranial infections in children must individualize the extent of initial surgical approach.…”
