Kidney stone disease is an urgent medical and social problem. Genetic factors play an important role in the disease development. This study aims to establish an association between polymorphisms in genes coding for proteins involved in calcium metabolism and the development of calcium urolithiasis in Russian population. In this case-control study, we investigated 50 patients with calcium urolithiasis (experimental group) and 50 persons lacking signs of kidney stone disease (control group). For molecular genetic analysis we used a previously developed gene panel consisting of 33 polymorphisms in 15 genes involved in calcium metabolism: VDR, CASR, CALCR, OPN, MGP, PLAU, AQP1, DGKH, SLC34A1, CLDN14, TRPV6, KLOTHO, ORAI1, ALPL, and RGS14. High-throughput target sequencing was utilized to study the loci of interest. Odds ratios and 95% confidence intervals were used to estimate the association between each SNP and risk of urolithiasis development. Multifactor dimensionality reduction analysis was also carried out to analyze the gene-gene interaction. We found statistically significant (unadjusted p-value < 0.05) associations between calcium urolithiasis and the polymorphisms in the following genes: CASR rs1042636 (OR = 3.18 for allele A), CALCR rs1801197 (OR = 6.84 for allele A), and ORAI1 rs6486795 (OR = 2.25 for allele C). The maximum OR was shown for AA genotypes in loci rs1042636 (CASR) and rs1801197 (CALCR) (OR = 4.71, OR = 11.8, respectively). After adjustment by Benjamini-Hochberg FDR we found only CALCR (rs1801197) was significantly associated with the risk of calcium urolithiasis development. There was no relationship between recurrent course of the disease and family history of urolithiasis in investigated patients. Thus we found a statistically significant association of polymorphism rs1801197 (gene CALCR) with calcium urolithiasis in Russian population.

Under the action of exogenous, androgenic, genetically determined factors, the metabolism of stone-forming salts of calcium, phosphorus, magnesium, oxalates, uric acid in the blood serum and their active excretion by the kidneys to the state of hypersaturation (oversaturation) is disturbed) urine is formed. When the level of crystallization inhibitors is disturbed, a saturated salt solution crystallizes with the formation of microliths. The formation of stones in the kidneys is possible only in the presence of «building material» – supersaturated saturated urine, therefore, hyperoxaluria is a pre-stone condition. Treatment measures should be aimed at correcting mineral metabolism in the body after establishing the type of hyperoxaluria using laboratory tests: salt transport, calcium load, dietary test – low-calcium diet, thiazide test and determination of the mineral composition of the removed (removed) stone. Genetically consequential conditions (10–15%) count about 30 varieties in which the main sign or symptom in the manifestation of the disease is urolithiasis. Unfortunately, congenital tubulopathies are not sufficiently studied, so the treatment is symptomatic, in some cases simultaneous kidney and liver transplantation options are possible. Clinically, 4 main forms of hypercrystalluria are distinguished: hypercalciuria, hyperoxaluria, hyperuricuria, phosphaturia and mixed forms of crystalluria. Acquired forms of hypercrystalluria, of which they are absorptive (type II intestinal hyperabsorption – absorptive hypercalciuria and absorptive hyperoxaluria), are of main clinical interest, which is characteristic of the course of calcium-oxalate urolithiasis. Metaphylaxis of calcium-oxalate urolithiasis is formed on the basis of these data.

Течение наиболее распространенного кальций-оксалатного нефролитиаза сопровождается гиперкристалурией, что является высоким риском рецидивирования мочекаменной болезни. Для проведения научно обоснованной метафилактики необходимо выделить виды кристаллизации. Нами предложены тесты: 1. Низкокальциевая проба применяется при гиперкальциурии. На протяжении 3 сут больному назначают низкокальциевую диету с последующим определением уровня кальция в суточной моче. Снижение уровня кальция до нормы свидетельствует о наличии абсорбтивной формы гиперкальцийурии II типа. Причиной является гиперабсорбция кальция из кишечника. Для проведения метафилактики необходимо обязательно рекомендовать больному низкокальциевую диету. в случаях отсутствия снижения гиперкальцийурии устанавливают диагноз гиперкальцийурии I типа (не абсорбционная), что связано с нарушением ферментативной активности печени. 2. Кальциевая проба (абсорбтивная гипероксалурия). Результаты исследования суточной мочи на содержание камнеобразовывающих солей свидетельствуют о 25-30% случаев значительного повышения уровня оксалурии (76-114 мг/л при норме 35-40 мг/л) при нормальном уровне кальцийурии. Гипероксалатурия является агрессивным фактором гиперкристаллизации, уровень которой зависит от использования диеты, абсорбции в кишечнике. Активность абсорбции оксалата зависит от уровня кальция в кишечнике, который связывает оксалат и переводит в нерастворимую форму, тем самым препятствуя абсорбции. Кальциевая проба: применение глюконата кальция 0,5 трижды в день в течение трех дней. При контроле уровня оксалатурии установлено снижение уровня оксалата в моче до верхних границ нормы (38-45 мг/л), что свидетельствует об абсорбтивной гипероксалурии. При метафилактике кальций-оксалатного нефролитиаза с выраженной гипероксалурией показана кальциевая диета. 3. Тиазидная проба проводится при повышенных показателях паратгормона с гиперкальцийурией и гиперурикемией. На протяжении 3-7 дней больному назначают тиазидные диуретики (гидрохлортиазид 50 мг 1 раз в сутки, или хлорталидон 50 мг 2 раза в сутки или хлортиазид 4 мг 1 раз в сутки) с последующим определением уровня кальция в крови и моче. При выраженной гиперкальциемии диагностируют резорбтивную форму гиперкальцийурии. Необходима консультация эндокринолога относительно резекции паращитовидных желез дальнейшего лечения. Дифференциальная диагностика гиперкальцийурии дает возможность научно обосновать противорецидивное лечение наиболее распространенного кальций-оксалатного нефролитиаза с высокой эффективностью и процентом (92-95%) безрецидивного периода в течение 5 лет метафилактики.
