Early Detection of High-grade Biliary Intraepithelial Neoplasia (BilIN-3) in the Cystic Duct Visualized by SpyGlass DS Cholangioscopy

Abstract: An 84-year-old man was admitted with epigastralgia. Computed tomography showed contrast-enhanced wall thickness in the cystic duct. An endoscopic examination revealed short irregular stricture in the cystic duct, and per-oral cholangioscopy revealed a reddish papillary tumor at the stricture site. Surgical resection revealed high-grade biliary intraepithelial neoplasia (BilIN) at the stricture site of the cystic duct. To our knowledge, this is the first case of a solitary high-grade BilIN epithelium in the cys… Show more

“…MRCP combined with CA 19-9 every 6–12 months has been shown to have a sensitivity of up to 100% and is a popular surveillance method worldwide, though it currently lacks evidence 11 12. Imaging modalities such as endoscopic ultrasound or endoscopic retrograde cholangiopancreatography have also been described in the detection of cystic duct carcinoma, made more specific when combined with brush cytology 11 13. The latest development would be cholangioscopy (SpyGlass), which has been described in the early detection of HGD (BilIN-3) 9.…”

“…They had a good experience with an 84-year-old male patient with epigastralgia. 29 There is no uniform standard for the interpretation of visual impressions gained from POCS of malignant bile duct strictures. It is currently generally accepted that, microscopically, irregular tortuous dilatation of blood vessels, polypoid masses, and fish-egg-like lesions are closely related to malignant tumors.…”
