Estudo comparativo entre a porca convencional e a porca para fixação óssea (PFO) ximais do úmero), sendo oito masculinos e oito femininos, com média de idade de 60,56 anos. Em cada um dos 16 cadáveres, o úmero de um dos braços foi fixado com sistema de placa-parafuso-PFO e o contralateral, com o sistema placa-parafuso-porca de metal convencional. Foi aplicada uma força sobre os sistemas, medida por meio de torquíme-tro, até que um dos elementos falhasse (porca de polietileno, porca convencional ou osso). Depois, foi medida a força usada individualmente por 20 ortopedistas para apertar parafusos até que acreditassem que o sistema podia ser considerado como estável. Todos os resultados foram anotados e avaliados estatisticamente. Resultados: Na avaliação da força média utilizada para aperto de parafusos, nas fixações rotineiras, individualmente, por 20 ortopedistas experientes, registrou-se a força média de 3,18N.m, força esta inferior à resistência do sistema placa-parafuso-PFO, que foi de 6,37N.m. A falha do sistema no osso com a PFO ocorreu em três casos e do sistema no osso com a porca de metal, em 10. Conclusão: A porca PFO mostrou-se menos agressiva ao osso que a porca metálica convencional, apresentando menor índice de fratura no osso. A força média aplicada pelo ortopedista não é capaz de quebrar a porca e levar a falha do sistema.

“…We did not observe any incidences of avascular necrosis in our series, however the literature reports an incidence rate of 4% to 75%, this could be due to the smaller duration of follow up in our study, a longer follow up time may yield incidence of this complication. 17,21,24 The most important factors in humeral head ischemia post fracture according to Hertel et al, are the dorsomedial extension at the metaphysic (less than 8mm), medial hinge integrity (a displacement of more than 2mm of the shaft) and fracture with the 2,9,10,11 and 12 (binary description system) components of the anatomic neck of the humerus. The positive predictive value of the ischemia is 97%, upon the presence of all three of the afore mentioned criterion.…”

ABSTRACT… Objectives:The aim of our study is to study the radiological and functional outcome of proximal humerus fractures treated via open reduction and internal fixation using the proximal humerus internal locking system or PHILOS. Study Design: Case series study. Period 05 years duration from January 2011 to December 2015. Setting: Large tertiary care centre in Karachi, Pakistan. Materials and methods: The study population consisted of n=50 patients all of whom underwent open reduction and internal fixation utilizing the proximal humeral internal locking system or PHILOS for fractures of the proximal humerus. The inclusion criterion was all the patients with closed fractures of the proximal humerus and were belonging to 2,3 and 4 part of the Neer system of classification. Physiotherapy was started as soon as possible for the patients. Serial radiographic imaging in two views was done at 6, 12, 24 and 52 weeks postoperatively. For the functional outcome of the procedure Constant and Murley scoring system was used. Data was analyzed using IBM SPSS version 21. Results: The study population consisted of n= 50 patients of which n= 35 were males and n= 15 were females having a mean age of 38.50 years. The mean duration of follow up was 24 months. All the patients in the study had union of fracture both radiographically and clinically, the mean time duration for the radiographically evident union of the humerus bone was 12 weeks with a range of 8 to 20 weeks, the mean Constant Murley score for the functional outcome of the shoulder joint was 79 at the final follow up with a range of 50 to 100. Complications were found in n= 9 patients and varus malunion was the most common complication. In our case series we did not observe complications such as avascular necrosis, non union or implant failure. Conclusion: According to the results of our study the proximal humerus internal locking system or PHILOS is a good method for open reduction and internal fixation of the proximal humerus fractures and provides a stable fixation, and has lower incidence of complications such as avascular necrosis. Background Locking plates have become a commonly used fixation device in the operative treatment of three-and four-part proximal humerus fractures. Examining function in patients treated nonoperatively and operatively should help determine whether and when surgery is appropriate in these difficult-to-treat fractures. Questions/purposes We compared functional scores, ROM, and radiographs in patients with one-part proximal humerus fractures treated nonoperatively to those in patients with displaced three-and four-part proximal humerus fractures treated with open reduction and internal fixation using locking plates. Patients and Methods We retrospectively reviewed 142 patients with proximal humerus fractures treated with a standardized treatment algorithm over a 6-year period.
