Early and long-term comparison of endovascular treatment of iliac artery occlusions and stenosis

Abstract: In our experience, endovascular treatment of iliac occlusions provides excellent early and long-term results, similar to those obtained in the treatment of stenotic lesions.

“…Also, in our previous report on endovascular management of iliac stenosis and occlusion, we found an increased risk of thrombosis in female patients at univariate analysis, even if this rate was not statistically significant. 15 In this study, we have analyzed the effect of gender on the results of endovascular procedures in patients with infrainguinal disease, and, to the best of our knowledge, there is only one other article in the literature making such an analysis. In fact, DeRubertis et al 7 compared the results of 447 endovascular procedures performed in women with those obtained in 553 procedures performed in men.…”

“…Comorbidities and risk factors were defined as previously described. 2,7 The aortoiliac-femoral lesion was defined according to the TASC II classification, and the clinical status was defined according to the Rutherford classification. 15 Early (intraoperative and <30 days) results were analyzed in terms of death, thrombosis, reinterventions, and occurrence of major local and systemic complications defined according to recommended standards for reports dealing with lower extremity ischemia.…”

“…[1][2][3] In particular, the "kissing stent" technique has been proposed for the treatment of complex aortoiliac lesions; it is particularly attractive for patients with high surgical risk because complications and mortality of AbF bypass still remain significant issues. [4][5][6][7] Moreover, in more complex disease patterns, such as those with the obstruction extending beyond or those with an associated severe femoral artery disease, hybrid technique using femoral artery endarterectomy in combination with aortoiliac kissing stents has emerged to extend the applicability of an endovascular approach.…”

“…If crossing the lesion through a retrograde approach is not successful, brachial access can be later obtained for an antegrade approach. The need for brachial access in iliac occlusions has been reported as 30% as compared to 4–5% with stenosis, resulting in similar technical success rates [36]. …”
