Dental implants are considered the first choice to replace lost or non-restorable teeth. However, the posterior maxilla remains a challenge in its management because of the quality of bone in the posterior maxilla. Osseo densification (OD) concept has been proposed in the literature to improve primary implant stability, which is an important aspect of osseointegration. Densah bur is novel drills specially designed to enhance a bone density by Osseo densification, which in turn increases primary stability. This present study was conducted to assess crestal sinus floor elevation by osteotome in comparison to Densah bur in the posterior atrophic maxilla. This was a randomized controlled clinical trial conducted on 20 patients to evaluate available crestal bone height loss, implant stability after implant placement in healed posterior maxillary alveolar ridge, whole bone height, Schneiderian membrane trauma, and post-operative complication.

Purpose: To evaluate the influence of 2 different loading protocols on single implant supported overdentures, immediate and conventional protocols on the supporting structures regarding: The extent of resorption of peri-implant bone and clinical evaluation of peri-implant soft tissue. Material and Methods: ten completely edentulous patients was selected to receive single implant supported overdenture. They have been divided after the insertion of implant into two groups according to time of loading. Group I received ball and socket attachment and loaded after 3 months of surgical procedures, while Group II was loaded within 7 days from surgery. Then all patients were evaluated clinically regarding probing depth, bleeding and plaque index and radiographically at the day of loading, 3, 6 and 9 months postoperatively. Results: comparison between the two groups in marginal bone loss was insignificant except in the first two periods, while probing depth was insignificant except at (6 to 9 month). For bleeding and plaque index, it was insignificant except at time of insertion for bleeding index. Conclusions: Immediate loading of single implants supporting a ball-retained mandibular overdenture is associated with more marginal bone resorption when compared with delayed loading of implants after 9 months follow up but it still can be used with single implant supported overdenture. Clinically, immediate loading had probing depth and bleeding index higher than delayed loading, while for plaque index for both loading protocols was almost the same.

Finite Element Analysis (FEA) of Palatal Coverage on Implant Retained Maxillary Overdentures

Purpose: The aim of this study was to determine stress levels on supporting structures of implant-retained overdentures as a function of varying degrees of palatal coverage using finite element analysis modeling at different loading angles. Materials and Methods: ABAQUS®-software was used to perform finite element analysis on eight overdenture models with three and four implants and with and without palatal coverage designs. Loads were applied perpendicular and 45º to the implants. Von Mises stress was measured to determine bone stress. A one-way ANOVA determined which model caused the most stress to the maxillary bone. Results: Palatal coverage increased stress to anterior implant in three implant (p = 0.08) models but decreased stress to all implants in four implant models (p = 0.43). Distal implants received more stress than anterior implants for all models. There was no significant difference between a full palate and no palate coverage overdenture prosthesis when a bar was added under axial loading (p = 0.954). Under non-axial loading, a decrease in stress was noted with the bar in all areas except the anterior implant site. Conclusions: Palatal coverage may not be necessary when applying a pure axial load. The addition of a bar decreased stress at loading.
