Objective. To appraise the cost-effectiveness of competing therapeutic strategies in patient cohorts eligible for aspirin prophylaxis with varying degrees of gastrointestinal (GI) and cardiovascular risk. Methods. Cost-effectiveness and cost-utility analyses were performed to evaluate 3 competing strategies for the management of chronic arthritis: 1) a generic nonselective nonsteroidal antiinflammatory drug (NSAID NS ) alone; 2) NSAID NS plus a proton pump inhibitor (PPI); and 3) a cyclooxygenase 2-selective inhibitor (coxib) alone. Cost estimates were from a third-party payer perspective. The outcomes were incremental cost per ulcer complication avoided and incremental cost per quality-adjusted life year (QALY) gained. Sensitivity analysis was performed to evaluate the impact of varying patient GI risks and aspirin use. Results. In average-risk patients, the NSAID NS ؉ PPI strategy costs an incremental $45,350 per additional ulcer complication avoided and $309,666 per QALY gained compared with the NSAID NS strategy. The coxib strategy was less effective and more expensive than the NSAID NS ؉ PPI strategy. Sensitivity analysis revealed that the NSAID NS ؉ PPI strategy became the dominant approach in patients at high risk for an NSAID adverse event (i.e., patients taking aspirin with >1 risk factor for a GI complication). Conclusion. Generic nonselective NSAIDs are most cost-effective in patients at low risk for an adverse event. However, the addition of a PPI to a nonselective NSAID may be the preferred strategy in patients taking aspirin or otherwise at high risk for a GI or cardiovascular adverse event.

Um grande número de pacientes na prática clínica apresenta-se com sintomas que podem ser atribuídos à região gastroduodenal. Estes incluem dor epigás-trica e queimação, saciedade precoce, plenitude pós--prandial, arrotos excessivos e náuseas ou vômitos. Sintomas epigástricos são descritos na região entre o umbigo e a extremidade inferior do esterno, e limitados pela linha médio-clavicular. Plenitude pós--prandial é uma sensação desagradável de persistência prolongada de alimentos no estômago. Saciedade precoce é a sensação de que o estômago está cheio logo depois de iniciar-se a refeição, em desproporção com o tamanho da mesma, de tal modo que não possa ser terminada. Dor epigástrica refere-se à sensação mais intensa e desagradável de danos aos tecidos. Se a dor epigástrica tem uma qualidade de ardor, é descrita como queimação epigástrica. Náusea é definida como mal estar acompanhado de um sentimento da necessidade de vomitar. Vômito é a expulsão forçada por via oral do conteúdo gástrico associada à contração dos músculos da parede abdominal e do tórax. Geralmente é precedido por náuseas e frequentemente associado com ânsia (contrações repetitivas da parede abdominal sem expulsão de conteúdo gástrico) (1) . O vômito é diferente de regurgitação ou ruminação em que o material não pode jamais ser contido na boca voluntariamente.Quando estes sintomas são crônicos e ocorrem na ausência de uma doença orgânica que prontamente os explica, o paciente é considerado como tendo uma desordem funcional gastroduodenal (1) . Estes distúrbios são classificados como dispepsia funcional (DF), eructações, distúrbios funcionais com náuseas ou vômitos, e a síndrome de ruminação.DF é definida como a presença de sintomas que se originam na região gastroduodenal (saciedade precoce, plenitude pós-prandial, dor epigástrica ou ardor), na ausência de qualquer doença orgânica, sistêmica ou metabólica susceptível de explicar os sintomas. DF é subdividida em duas categorias de diagnós-tico: sintomas dispépticos induzidos por refeição com síndrome do desconforto pós-prandial (SDP), caracterizada pela plenitude pós-prandial e saciedade precoce e síndrome de dor epigástrica (SDE), caracterizada por dor epigástrica e queimação (1) . Náuseas e vômitos podem acompanhar uma complexa variedade de distúrbios orgânicos gastrointestinais e doenças sistêmicas. Quando estes podem ser excluídos como causa, o paciente pode ser considerado como tendo um distúrbio funcional com náuseas e vômitos. Distúrbios funcionais caracterizados pela presença de náuseas inexplicadas, crônicas, com ou sem vômitos, podem ser subdivididos em náusea idiopática crônica, vômito funcional, ou síndrome de vômitos cíclicos (1) .
