ök hücre nakli (KHN); kemik iliği hasar görmüş hastalarda kemik iliği fonksiyonunun yeniden yapılandırılması amacıyla intravenöz (IV) ya da oral yüksek doz kemoterapi veya radyoterapiyi takiben Hematopoetik Kök Hücre Nakli ve Güncel Beslenme Yaklaşımları Ö ÖZ ZE ET T Hematopoetik kök hücre nakli (KHN); lösemilerin, solid tümörlerin, bazı hematolojik, nö-rolojik, metabolik ve otoimmün hastalıkların tedavisinde kullanılmaktadır. Hastanın kendi kök hücrelerinin kendisine verilmesiyle (otolog kök hücre nakli) veya başka bir vericiden hastaya kök hücre transfer edilmesiyle (allojeneik kök hücre nakli) yapılabilmektedir. KHN sonrası, donörden alınan kök hücrelerle birlikte verilen sağlıklı T-lenfositlerin aracılık ettiği, şiddetli immünolojik reaksiyon sonucu görülen organ disfonksiyonu ile karakterize, ortaya çıkış zamanına göre akut veya kronik seyreden graft-versus-host-hastalığı (GVHH) gelişebilmekte ve bu durum en önemli mortalite ve morbidite sebebi olarak kabul edilmektedir. Hastalarda KHN sonrası görülen komplikasyonları (kusma, diyare, oral mukozit, anoreksiya) azaltmak, iyileşme sürecini hızlandırmak, hematopoezi desteklemek, doku reddini ve GVHH'ı önlemek için beslenme durumunun saptanması ve beslenme desteğinin sağlanması gerekmektedir. Bu amaçla oral alımın sağlanamadığı ya da yetersiz sağlanabildiği durumlarda enteral, total parenteral ya da kombine beslenme seçenekleri değerlendirilmeli, hastanın enerji, protein, lipit, karbonhidrat, vitamin ve mineral gereksinimleri sağlanmalıdır. Son dönemde bu hasta grubunda yapılan çalışmalarda, omega-3 yağ asitleri, glutamin, D vitamini ve prebiyotik/probiyotiklerin olumlu etkileri bildirilmektedir.A An na ah ht ta ar r K Ke el li im me el le er r: : Hematopoetik kök hücre transplantasyonu; beslenme tedavisi; enteral beslenme; parenteral beslenme; graft vs host hastalığı A AB BS ST TR RA AC CT T Hematopoietic stem cell transplantation (HSCT) which is used in the treatment of solid tumors, some hematologic, neurologic, metabolic and autoimmune diseases can be operated autologous (involving tissues or cells that are genetically similar) or allogeneic (involving tissues or cells that are genetically dissimilar). Healthy T-lymphocyte-mediated acute or chronic graft-versus-host-disease (GVHD) according to stem cells taken from a donor can result in severe immunological reaction characterized by organ dysfunction may be seen after HSCT. This complication is considered as the most important cause of morbidity and mortality. Determining nutritional status and improving by nutritional support is essential in order to reduce complications (vomiting, diarrhea, oral mucositis, anorexia), accelerate the healing process, prevent rejection, support emerging hematopoiesis and prevent GVHD. For this purpose, total parenteral or enteral nutrition or combined options should be considered when oral feeding is not enough.
