On models of chronic neurogenic pain (CNP) and the growth of a malignant tumor (metastasizing B16 melanoma) in male mice, we studied an effect produced by mitochondrial therapy (MCT) on the state of the myocardium. Some structural correlates of the compensatory-restorative effect by mitochondria transplanted from healthy recipient rats were revealed. It has been identified that MCT contributes to the preservation of the structural integrity of the myocardial tissue, the inclusion of an auxiliary link in the cellular mechanisms of tissue restoration: fibroblasts, histiocytes, lymphocytes, eosinophils and other connective tissue elements, which implement the intercellular mechanism of information transfer that provides the external regulatory function of MCT. The ability of mitochondria to prevent the DNA decay determines the possibility of initiation of the operation of the nuclear mechanisms of the cardiomyocyte division, which is characteristic of a population of young cells and which indicates the determining position of exogenous mitochondria.

РезюмеАктуальность. Несмотря на функциональную множественность и сложность действий TNF-α, его роль в центральной нервной системе при развитии различных патологических состояний в организме до сих пор не понята. Цель. Изучить динамику TNF-α и его рецепторов в головном мозге мышей с нокаутом по урокиназе (uPA-) при злокачественном росте на фоне хронической нейрогенной боли (ХНБ). Материалы и методы. Работа выполнена на мышах обоего пола двух линий: С57ВL/6 (n = 80) и C57BL/6-PlautmI.IBug-ThisPlau6FDhu/GFDhu (n = 56). В основных группах животных ХНБ моделировали путем двусторонней перевязки седалищных нервов, через 2 недели под кожу спины перевивали меланому В16/ F10. Группа сравнения -ложно оперированные животные с перевивной меланомой. Контрольные группы -ложно оперированные животные и животные с ХНБ. На 21 сутки роста меланомы мышей декапитировали, в церебральной ткани методом иммуноферментного анализа определяли содержание TNF-α, sTNF-α R1 и sTNF-α R2. Результаты. В мозге самцов uPA-регистрировался высокий уровень TNF-α (в 5,6 раза) и низкий -TNF-α R1 (в 2,1 раза), у самок -изменений не было. Под влиянием ХНБ содержание TNF-α в мозге становилось одинаковым у самцов и самок мышей, при этом у мышей uPA-уровень TNF-α был более чем в 2,0 раза больше, чем у мышей с нормальным геномом. Основу стимулирующего действия ХНБ на злокачественный процесс у мышей uPA-составили: у самок -увеличение церебральной концентрации TNF-α R2 (в 1,7 раза; р < 0,05), у самцов -редукция TNF-α (в 2,2 раза). Заключение. Специфика фибринолитического статуса мышей определяет течение злокачественного процесса подкожно перевитой меланомы на фоне боли, один из механизмов этого явления -изменение функционирования системы TNF-α головного мозга, зависящее от пола животного.

The development of a new direction in anticancer medical therapy – the use of immune checkpoint inhibitors targeting PD-1/ PD-L1 and CTLA-4 – has significantly changed the approach to tumor treatment in the last few years. The PD1 blocker nivolumab in major registered clinical trials improved overall survival, including in metastatic melanoma, with a favorable toxicity profile. However, its efficacy in patients with brain metastases from melanoma was poorly studied, since the inclusion criteria for most clinical trials do not envisage recruiting such patients. The immune-mediated toxicity of immune checkpoint inhibitors is currently well enough studied. However, cases of cutaneous toxicity are quite rare and present certain difficulties for differential diagnosis and treatment. This article presents two cases of effective nivolumab treatment in patients with generalized BRAFwt and BRAFmut cutaneous melanoma. The first case is of interest due to the presence of brain metastases in the patient. Nivolumab therapy helped achieving complete regression of intracranial metastases with the long-term effect. The second case, in addition to effective treatment, demonstrates a rare manifestation of skin toxicity – vitiligo on the face and upper extremities.
