Our knowledge of the role of muscle activation on proprioception is incomplete. Previous work has either focused on comparing active and passive motions or manipulated both muscle activation and joint angles simultaneously. We conducted an experiment at the shoulder in which subjects' trunks were tilted backwards to decouple joint angle from joint torque. Twenty three healthy subjects underwent testing in an unconstrained joint position sense task. Kinematics were measured with a magnetic tracking device. The joint position sense task consisted of subjects moving their arms to a predetermined orientation in space with the help of visual feedback from the magnetic tracking device presented to the subjects through a head-mounted display. Subjects were then asked to reproduce the presented joint position in the absence of visual feedback. The protocol was performed under two tilts: upright and trunk tilted back 458. This allowed for a comparison of joint position sense at different joint angles (at the same resistive torque) and at different resistive torques (at the same joint angles). When comparing these two tilts, we found that matching based on elevation angle demonstrated no significant difference, while matching based on torques did find differences. These results implicate elevation angle at the shoulder as playing a more important role in modulating joint position sense than joint torque. ß

Artroskopinin gelişmesi sonrası bu tip instabilitelerde de eklem içinde lezyonlar saptanması, bu sınıflamanın yetersiz olduğu görüşünü kuvvetlendirmiştir. Patolojileri tanımlamak-ta yetersiz kalsa da bu sınıflama, klinik kullanım kolaylığından dolayı, uzun yıllar popülerliğini korumuştur.Artoskopinin yaygınlaşmasına paralel olarak 2000'li yıllarda, instabilitenin nedeni, yönü, çıkık sayısı gibi birçok parametreyi esas alan yeni sınıflamalar yapıl-mıştır. Gerçekten bu sınıflamalar, omuz instabilitelerinin ayrıntılarıyla tanımlanması açısından çok beğeni toplamıştır. Ancak bu sınıflamaların, instabiliteyi çok iyi tanımlamış olmalarına karşın, tedavide yol gösteri-cilikleri halen yeterli değildir. TARİHÇETravmatik omuz çıkıkları, Mısır ve Yunan tıbbında çok iyi bilinen en eski omuz sorunlarıdır. Ancak çok yönlü instabilite (ÇYİ) 1980 yılında Neer ve Foster [1] tarafın-dan tanımlanmıştır. Bankart ve Perthes, instabil omuzdaki lezyonları 20. Yüzyıl başında tanımlamışlardır. Uzun yıllar instabilite bu lezyonlar ile ilişkilendirilerek tarif edilmiştir. ÇYİ'den sorumlu tutulan eklem kapsülü gevşekliği ise, 1980 yılında bir patoloji olarak omuz instabilitesi nedeni olarak kabul edilmeye başlanmıştır. [2,3] Omuz instabilitesinin sınıflamasında çok uzun yıl-lar Matsen ve arkadaşlarının TUBS ve AMBRI olarak kodladıkları sistem kabul edilmiştir. AMBRI temelde çok yönlü instabiliteyi işaret eder. Travma olmaksı-zın geliştiği kabul edilen bu tip instabilitenin eklem içi Multidirectional instability of the shoulder is a complex problem because of intra articular and extra articular pathologies. This condition of the shoulder can be a difficult diagnostic and therapeutic dilemma for the shoulder surgeons. The exact diagnosis can be achieved with patient's history and special physical examinations. Multiple factors need to be considered before the treatment of multidirectional instability is instituted, including the age, activity level, bone and soft-tissue pathologies, degree, direction, frequency, and etiology of instability. Treatment options can be physical therapy, arthroscopic or open surgery. Operative treatment is indicated when effective non-operative treatment has failed. Arthroscopic capsulolabroplasty is a reliable procedure, which not only provides capsular plication, but also detects and treats other intraarticular pathologies of the instable shoulder such as Superior Labrum Anterior Posterior and posterior labral tear. InquadramentoQuando si parla degli aspetti clinici dell'instabilità multidirezionale della spalla (IMD) è importante fare distinzione tra la "lassità" e l'"instabilità". Per lassità gleno-omerale si intende una condizione congenita e asintomatica in cui si riscontra ampia mobilità dell'articolazione stessa, legata a una maggior elasticità dei tessuti capsulo-legamentosi. Al contrario, l'instabilità è la perdita di contatto articolare (sublussazione o lussazione) dell'epifisi omerale rispetto alla glena, associata a sintomi quali dolore, apprensione e deficit funzionale. Esistono numerosi modi di classificare le instabilità gleno-omerali [1] prendendo in considerazione la frequenza, la direzione, l'eziologia e la possibilità di causare volontariamente una sublussazione o una lussazione. Con IMD si identificano quei pazienti che lamentano instabilità recidivante in almeno una direzione, associata a una lassità multidirezionale apprezzabile all'esame obiettivo. Sono pazienti fra 20 e 30 anni, che per lo più lamentano dolore senza una chiara anamnesi di lussazioni traumatiche. Tali pazienti presentano una lassità multidirezionale della spalla alla quale si associa un'instabilità unidirezionale, a seguito di un trauma o, come più spesso accade, di microtraumi ripetuti. L'attività sportiva intensa svolge un ruolo determinante nella patogenesi, richiedendo ai giovani atleti ripetuti sforzi nelle posizioni estreme della rotazione interna/esterna in abduzione/elevazione (per es., sport di lancio, nuoto, pallavolo ecc.). Esistono i cosiddetti eventi precipitanti che trasformano una spalla lassa in una spalla instabile e solitamente sono eventi legati più all'utilizzo quotidiano dell'arto piuttosto che a un trauma importante; a volte l'evento non può essere identificato. Il dolore è ricorrente, avvertito durante le attività sportive/lavorative (già svolte in passato senza problemi), spesso non ai massimi gradi del movimento ma durante l'esecuzione del movimento stesso. Il dolore ha le caratteristiche della trafittura, non di lunga durata, e il paziente difficilmente riesce a evocarlo o a descriverlo con precisione. Comuni sono la sensazione di perdita della forza e la "dead arm syndrome" notturna, entrambi sintomi fastidiosi e difficilmente obiettivabili che si associano nel tempo al dolore. Qualora in anamnesi siano presenti episodi di lussazione è bene saperne il numero, le modalità di presentazione e della riduzione.
