Dynamic needs and challenges of people with drug-resistant tuberculosis and HIV in South Africa: a qualitative study

Abstract: Summary Background There is little evidence of patient acceptability for drug-resistant tuberculosis (DRTB) care in the context of new treatment regimens and HIV co-infection. We aim to describe experiences of DRTB-HIV care among patients in KwaZulu-Natal province, South Africa. Methods In this qualitative study using Bury’s framework for chronic illness, we conducted 13 focus groups at a tertiary hospital with 55 patients co-infected with DRTB and HIV (28 … Show more

“…With our model of care, we aimed to reduce the period that patients were separated from their family and home and the need for travel in an insecure environment. However, decentralised RR‐TB care would have brought care closer to their homes and would have had fewer social and economic consequences [ 21 , 22 ]. Interventions to mitigate economic loss should be integrated into any RR‐TB control strategy.…”

“…Our results show that travelers coming from Western Pacific region have the largest number of detected infectious cases, which can be attributed to this region being responsible for the largest number of travelers coming to China. Furthermore, the travelers entering Shanghai Port of China from Africa have the highest detection rate of infectious diseases, which may be attributed to the lack of local health resources and the relatively higher infection rate in Africa ( 31 ). These findings suggest that in the context of international exchanges, the prevention and control of infectious diseases endemic to Africa is an indispensable step in the prevention of global spread of these diseases.…”

“…[ 29 ] Ten percent of these patients develop resistance mutations to fluoroquinolones and second-line injectable drugs due to improper treatment, which leads to extensively drug-resistant TB (XDR-TB). [ 30 ] DR-TB treatment is more complex, has a higher failure rate, and is more expensive than susceptible TB. The global treatment success rate for MDR/rifampicin-resistant TB in 2020 is 59%.…”
