Duodenum‐preserving versus pylorus‐preserving pancreatic head resection for benign and premalignant lesions

Abstract: DPPHR for benign or premalignant lesions is a difficult procedure with a higher complication rate than PPPD, but was without mortality. Preserving the entire duodenum and a normal biliary tree allows better long-term results.

“…In this series, our complication rate after RA‐DPPHR was high when compared to the complication rate after RA‐PD (47.1% vs. 32.4%) and other series (47.1% vs. 38.2%) . Different surgical procedures and definitions of complications might partly account for the difference in postoperative morbidity in these other published articles . The significantly high frequency of complication rates could be related to the learning curve.…”

“…Organ‐preserving procedures, such as Beger's procedure, are performed as an alternative to maintain the normal anatomy of the upper digestive system and reduce the risk of postoperative exocrine and endocrine insufficiency . In recent years, DPPHR has become an important treatment option for benign or premalignant pancreatic head tumours . Nevertheless, there is little evidence about short‐term and long‐term outcomes of minimally invasive DPPHR, especially regarding the robot‐assisted approach.…”

“…Pancreaticoduodenectomy (PD) has been performed in the treatment of a considerable number of malignant diseases such as pancreas, duodenum, ampullary and distal bile duct cancer and some benign diseases [1][2] . In recent decades, several studies have been conducted to evaluate clinical and nutritional longterm effects after PD [3][4][5][6][7][8] .…”
