Dumbbell-shaped thoracic hibernoma: computed tomography and magnetic resonance imaging findings

Abstract: Hibernoma is an uncommon, benign soft tissue tumor that arises in brown adipose tissue. The computed tomography (CT) and magnetic resonance imaging (MRI) findings of hibernomas are similar to those of well-differentiated liposarcoma or angiolipoma. We report the unique appearance of a rare thoracic wall hibernoma, which appeared as a dumbbell-shaped lipomatous tumor across an intercostal region. A dynamic contrast-enhanced study on MRI revealed early enhancement, which corresponded to the branching low-signal … Show more

“…Microscopically, the tumor is characterized by multivacuolated cells with eccentric nuclei and granular eosinophilic cytoplasm, univacuolated cells with peripheral nuclei, and smaller round cells with granular cytoplasm. The hypervascularity and the presence of cells with eosinophilic granular cytoplasm full of mitochondria give hibernomas their brown color [4,6,18]. From an histological point of view this entity must be distinguished from granular cell tumor, that is a benign peripheral nerve derived tumor composed of granular cells rich in mitochondria.…”

“…Histopathologic evaluation of hibernomas, as previously described, is well-established and pathognomonic. On the contrary, CT and MRI features are not specific and vary with the nature and amount of lipid component [4,12,18,19,22,23]. Non contrast CT usually demonstrates a well-demarcated soft tissue mass of predominantly low attenuation which is close but not identical to subcutaneous fat.…”

“…Esta última, con algunas secuencias particulares de supresión grasa, aporta datos concluyentes para la identificación del componente graso. 4,[7][8][9] En los hibernomas, la TC demuestra lesiones bien definidas, cuando su localización es muscular. Se observa realce difuso con contraste endovenoso.…”

“…Welldifferentiated liposarcoma resembles it on low-power examination, but closer inspection shows scattered tumor cells with large, deep-staining nuclei. [20] Due to this it is especially important to perform a differential diagnosis with lipoma, fibroma, neurofibroma, angiolipoma, and even with liposarcoma, by clinically and radiologically studies, making a differential diagnoses with myxoid liposacorm, angiomyolipoma and mesenteric panniculitis; [12] so myxoid liposarcoma may present a clinical picture and radiological findings similar to hibernoma and lipoblastoma, but it is associated with infiltrating growth, presenting cellular atypia and is extremely infrequent in the pediatric age; [20] the angiomyolipoma is a vascular tumour with striated muscle cells and adipose tissue, [18,21] and characteristically found at the renal level.…”
