Dual-Isotope 99mTc-MIBI/123I Parathyroid Scintigraphy in Primary Hyperparathyroidism

Abstract: In preoperative assessment of primary hyperparathyroidism and to guide surgery, we propose to perform first subtraction SPECT/CT and to complete it with neck pinhole, only if tomoscintigraphy is negative.

“…Preoperative detection of an ectopic adenoma on SPECT/CT is unquestionably of value prior to surgery (278). SPECT/CT provides accurate assessment in patients with coexisting thyroid nodular disease (240,279,280), particularly when combined with neck ultrasound (280). However, for patients with multiglandular disease or double adenomas, and for those undergoing reoperative surgical procedures the sensitivity of SPECT/CT appears lower (246,281,282), when compared to selective venous sampling (281), and therefore it is not expected to have a major impact for these more challenging patient subgroups.…”

“…Up to date, multiple different scintigraphic and SPECT-based methodologies for detecting pathological parathyroids have been described, including dual-phase, dual-isotope, planar and CT combined imaging approaches, with wide range of sensitivity and specificity for each method [4, 6, 1114]. Different approaches in calculation of sensitivity and specificity were used with different references as gold standard.…”

“…SPECT/CT dual-tracer MIBI/ 123 I parathyroid scintigraphy was also tested by Hassler et al [41] in comparison with pinhole planar scanning. Each parathyroid uptake was allocated to a neck parathyroid site (left superior, left inferior, right superior, right inferior) or classed as ectopic.…”
