Dual-energy contrast-enhanced spectral mammography (CESM)

Abstract: Dual-energy contrast-enhanced mammography is one of the latest developments in breast care. Imaging with contrast agents in breast cancer was already known from previous magnetic resonance imaging and computed tomography studies. However, high costs, limited availability-or high radiation dose-led to the development of contrast-enhanced spectral mammography (CESM). We reviewed the current literature, present our experience, discuss the advantages and drawbacks of CESM and look at the future of this innovative … Show more

“…Indeed, CESM could be immediately available in the mammography suite with minimal loss of time [9]. Therefore, CESM might be an alternative cost-effective imaging method for MRI, especially when MRI availability is limited [9]. Contrast enhanced mammography is emerging as a combined application of full-field digital mammography with intravenous injection of iodinated contrast medium.…”

“…There is enthusiasm regarding the potential for CESM to be superior to conventional mammography, through the extended information obtained from contrast media with existing mammography capacity and experience, it could be possibly comparable with MRI, yet more easily accessible. Indeed, CESM could be immediately available in the mammography suite with minimal loss of time [9]. Therefore, CESM might be an alternative cost-effective imaging method for MRI, especially when MRI availability is limited [9].…”

“…46 Despite this notable improvement in sensitivity, the specificity for the probability of malignancy remains about the same as conventional mammography, which is approximately 50 to 68%. 46, 47 To address the relatively poor specificity, one needs to establish the correlation between the quantitative information from contrast-enhanced mammograms and the pathological parameters of malignant lesions. However, standard contrast-enhanced spectral mammography technique relies on the weighted log-subtraction of the dual-energy images.…”

“…Another application of such phantoms is in testing, evaluation and clarification of the role and the benefits of new x-ray modalities in comparative studies to actual daily routine. Breast tomosynthesis (Sechopoulos 2013), contrast-enhanced mammography (Francescone et al 2014, Daniaux et al 2015, as well as new imaging modalities to be potentially established like breast computed tomography (CT) (Lindfors et al 2008, Mettivier and Russo 2011, Kalender et al 2012, Sarno et al 2015, dual-energy breast CT (Badr et al 2014) and phase-contrast breast imaging (Bravin et al 2013, Auweter et al 2014, Sarno et al 2017a are being explored.…”
