O tratamento da alteração cromática em um único elemento dentário apresenta-se como um grande desafio ao clínico, sendo muitas vezes solucionada por meio de restauração direta com resina composta. No entanto, em alguns casos dependendo do grau dessa alteração, a restauração indireta feita por cerâmica é mais indicada por apresentar uma melhor longevidade, sendo menos susceptível a alteração de cor. O presente relato de caso refere-se ao escurecimento do dente 21, pós-traumático, com relato de repetidas restaurações de resina composta para estabelecimento estético dentário. O plano de tratamento proposto à paciente foi a confecção de uma faceta cerâmica de dissilicato de lítio, preparada através do sistema CAD/CAM. Todos os passos operatórios foram realizados cuidadosamente, sendo que o diagnóstico, o planejamento restaurador protético integrado com o laboratório e a execução no processo de cimentação foram fundamentais para o resultado da restauração indireta.

The CAD/CAM technology arose from the need to develop materials with better mechanical and optical properties that could be used for making monolithic restorations. Several materials have been used for milling indirect restorations in prefabricated blocks. Among them, lithium silicate reinforced with zirconia. Due to its recent introduction in the market, this case report aimed to present a detailed clinical protocol for the execution of a ceramic onlay of this material using CAD/CAM technology. A 57-year-old female patient sought care with extensive restoration in composite resin (BOMD) of tooth 15 maintained for almost two years. However, due to bruxism, constant dental tightening, the extension of the direct restoration and the need for improved esthetics, it was proposed to replace it with an indirect ceramic onlay restoration. Prophylaxis and choice of the color of the patient’s dental substrate were performed. Afterwards, the dental preparation was made and polished. Subsequently, the molding was made with addition silicone and the provisional restoration confectioned. Once the stone model was obtained, it was scanned and the ceramic onlay restoration was milled using CAD/CAM technology. Finally, the restoration was stained and cemented over the preparation. After 2 years, the restoration remained stable, with no evidence of color mismatch, marginal discoloration, marginal cleft, caries or fracture, proving the effectiveness of the treatment in this time interval. The correct indication of the ceramic material combined with the use of CAD/CAM technology facilitated the restorative process, restoring function and the esthetics desired by the patient.

Objectives: Digital smile design (DSD) is useful in planning multidisciplinary esthetic treatments. However, DSD requires clinician training and skill to ensure its effective use. The Digital smile design application (DSDapp) was recently developed, to facilitate such planning. The objective of this study was to illustrate the use of the DSDapp for esthetic planning in a clinical case that included periodontal plastic surgery and ceramic laminate veneers. Clinical considerations: An intraoral digital scan was performed, and a photograph was obtained using an iPad (frontal facial full smile). The images were analyzed using the DSDapp. All reference lines were inserted, and dental shapes predetermined by the app were superimposed on the photographs. A digital diagnostic wax-up was performed considering the plan created in the DSDapp. After 3D printing the wax-up, a mock-up transferred the planning to the oral cavity. Following this, the patient was referred to a periodontist for the periodontal plastic surgery. After the healing period, the teeth were prepared for computer-aided design/computer-aided modeling lithium disilicate ceramic laminate veneers. Conclusions: DSDapp use accelerated the initial planning steps. Smile planning can be performed during the clinical session with the patient's active participation. In addition, the DSDapp facilitated better communication within the multidisciplinary team. Clinical significance: The DSDapp relies more on intuition than on skill and training to execute the treatment plan. The DSDapp provides immediate feedback to the patient, offering greater predictability and helps monitor the planning through all the clinical stages.
