MRH is common after hospital discharge in older adults, but methodological inconsistencies between studies and a paucity of data on risk factors limits clear understanding of the epidemiology. There is a need for international consensus on conducting and reporting MRH studies. Data from large, multicenter studies examining a range of biopsychosocial risk factors could provide insight into this important area of safety.

Pharmacists are increasingly using medication therapy problem (MTP) metrics to monitor their clinical impact and demonstrate their value to health care organizations and commercial payers. Although the practice of documenting and resolving MTPs is well reported in the literature, there remains great diversity in how MTPs are categorized, documented, and reported. We conducted a review of the literature to gain key insights into the MTP classification systems, methods of MTP documentation, and MTP-related outcomes that are reported in the literature, as well as the elements of MTP documentation that are commonly employed in practice. Ninety-one articles were identified. These insights were used to develop a set of recommendations to help standardize the practice of MTP documentation in the profession. This review highlights the findings from these studies and outlines a set of recommendations for identifying, categorizing, documenting, and monitoring MTPs that can be applied across care settings. The recommendations include clearly defining the purpose for documenting MTPs; using an established, standardized MTP categorization framework and applying that framework consistently in practice; establishing a clear process for identifying and resolving MTPs; systematizing the documentation and reporting process; tracking the resolution of MTPs identified; and monitoring MTP data and assessing impact. MTP identification, documentation, and reporting are an important strategy for capturing, monitoring, and sharing the unique contributions of pharmacists in providing patient care. Pharmacy has long struggled to embrace a unified professional identity despite many leaders calling for such. With societal and scientific evolutions, the functions of the pharmacist have changed over time, often assuming multiple identity discourses.Pharmacy has also been seen as a very heterogeneous profession with product-and patient-focused practitioners, contributing to the lack of a unifying identity. This commentary argues that to advocate for and promote our profession we must confront the brutal facts and realize the true state of pharmacy practice. The fact is that all areas of pharmacy practice, both product-and patient-focused, have always shared a central theme which is and has been the pharmacist's professional identity: the prevention, identification, and management of medication therapy problems and their root causes as the medication specialist. Fully embracing this professional identity will help the profession assume a clear and unique role on the healthcare team in optimizing patient care and allow for meaningful practice-based research, grounded in theory, which will improve the education of currently practicing and future pharmacists. A firm professional identity will also allow for a seamless continuation between pharmacist education and practice across a lifetime, strengthening the pharmacy profession as a whole.
