Drug-Induced Pancreatitis

Abstract: Among adverse drug reactions, pancreatitis is often-ignored because of the difficulty in implicating a drug as its cause. The physician should have a high index of suspicion for DIP, especially in specific subpopulations such as geriatric patients who may be on multiple medications, HIV+ patients, cancer patients, and patients receiving immunomodulating agents.

“…Although prolonged remission may occur in early type 1 diabetes, this honeymoon period is unlikely to last 3 years. Atypical type 2 diabetes, characterized by ketosis at onset and subsequent remission, has been described in African patients but not in whites (1). Nonetheless, the negative antibodies and subsequent insulin independence in this case favor type 2 diabetes as the more likely diagnosis.…”

“…In two large reviews, however, vincristine and vinblastine have been classified in the last class as the lowest list group in terms of triggering acute pancreatitis alone (5,8). “…In another classification based on number of published case reports, it is in the lowest risk group (5,8). Although acute pancreatitis has been reported with protocols combining its liposomal form with other agents, in general doxorubicin alone has never been blamed.…”

“…The incidence of acute pancreatitis may reach up to 40% of HIV seropositive individuals a year, which is considerably higher than for the general population, that has an incidence of 2%. 35 Pancreatitis has been predominantly associated with the usage of nucleoside reverse transcriptase inhibitors (NRTIs) such as didanosine and stavudine. 36 Being used widely and frequently, NSAIDs are often associated with ADEs.…”
