Drug-eluting coronary stents – focus on improved patient outcomes

Abstract: The development of stent has been a major advance in the treatment of obstructive coronary artery disease since the introduction of balloon angioplasty. Subsequently, neointimal hyperplasia within the stent leading to in-stent restenosis emerged as a major obstacle in long-term success of percutaneous coronary intervention. Recent introduction of drug-eluting stents is a major breakthrough to tackle this problem. This review article summarizes stent technology, reviews progress of drug-eluting stents and discu… Show more

“…Although it is uncertain whether treating depression directly reduces mortality and morbidity rates, patients whose depressive symptoms do not improve may have an increased risk of recurrent cardiac events. 16,40 Moreover, considering that the prevalence of depression after PCI is as high as approximately 20% to 30%, 4,16,17,19,23,36 early intervention, including assessment of depression level and detection of potential depressive patients, should be performed during hospitalization. Nurses should also implement psychoeducational interventions to reduce psychological symptoms, such as depression, and manage cardiovascular risk factors by increasing patients' involvement of positive behavioral change.…”

“…3 The use of drug-eluting stents (DESs), in which medications such as sirolimus or paclitaxel are applied to bare-metal stents (BMSs), has been reported to reduce rates of restenosis or stent thrombosis compared with BMS. 4 Restenosis rates were reduced from 30%Y40% 4 to 5%Y10% at 6 months and to 15% or less within 12 months in patients who underwent PCI with DES. 4Y8 However, restenosis and stent thrombosis still pose clinical problems.…”

“…PCI is widely used in the treatment of CHD (Cheng et al, 2019;Dayoub et al, 2019). Although the cases of the patients with coronary artery restenosis and thrombosis can be reduced from 30-40 to 15% after 1 year of PCI, restenosis, and thrombosis are still serious clinical problems (Jaffery et al, 2011;Hofma et al, 2015). Therefore, controlling the risk factors of CHD is still necessary to prevent the occurrence of MACEs.…”

“…Вместе с тем, отмечено, что снижение КЖ в определенной степени связано и с неблагоприятным клиническим исходом [19]. Однако, на наш взгляд, критерием эффективности примененных процедур должно быть повышение КЖ не только при краткосрочном, но и при отдаленном наблюдении [15] [12,13,18,29]. Авторы из Нидерландов приводят результаты сравнения таких процедур реваскуляризации [26].…”
