Drain Failure in Intra-Abdominal Abscesses Associated with Appendicitis

Abstract: Approximately a quarter of patients will fail management of appendiceal abscess with percutaneous drain placement alone. Risk factors for failure are patient complexity, female sex, earlier drainage, and Hispanic race. Failure of drainage is associated with higher total charges and longer hospital stay; however, no change in the mortality rate was noted.

“…Nine cases underwent CT‐guided drainage in the present study, but two cases (22.2%) failed to complete the planned interval period before ILA. In a previous study, 25.4% of 2209 patients with AAM did not experience improvement after abscess drainage and subsequently required surgical treatment 28 . Whether abscess drainage can be performed is not an absolutely critical factor in determining the feasibility of ILA.…”

“…However, among the 9 cases of AAM underwent CT guided drainage in the present study, 2 cases failed to reach ILA (22.2%). In a previous report, Horn reported that 561 patients (25.4%) among 2209 cases of AM failed to improve by drainage and required surgical treatments [27]. Whether abscess drainage can be performed or not is not an absolutely critical factor in completing the ILA procedures.…”
