BackgroundIndoleamine 2,3-dioxygenase (IDO) has been reported to play a key role in placental development during normal pregnancy. However, the question of whether endometrial IDO expression affects in vitro fertilization (IVF) pregnancy outcomes remains unclear. The current study was undertaken to investigate whether there was any association between endometrial IDO expression and IVF treatment outcome.MethodsThis retrospective study was designed to compare pregnancy outcomes among women with different endometrial IDO expression levels under their first IVF treatment. A total of 140 women undergoing their IVF treatment were selected from January 2017 to December 2017. Endometrial samples were collected during mid-luteal phase before IVF cycle. The endometrial IDO expression levels were analyzed by immunohistochemistry, and compared between women who were pregnant or not. A logistic regression analysis was performed to determine the impact of endometrial IDO expression on live birth.ResultsNo significant differences in the endometrial IDO expression levels were found between women who had clinical pregnancy and those who failed (P>0.05). However, the endometrial IDO expression level was significantly higher among women who had live birth compared with those who had no live birth (P=0.031). Additionally, after adjusting for differences in maternal age, BMI and duration of gonadotropin stimulation, women with higher IDO expression level had an increased live birth rate (adjusted odds ratio [aOR] 2.863, 95% confidence interval [CI] 1.180-6.947). ConclusionsHigher endometrial IDO expression level during mid-luteal phase is associated with an increased live birth rate in women undergoing their first IVF treatment.

During pregnancy, tryptophan (Trp) is required for several purposes, and Trp metabolism varies over time in the mother and fetus. Increased oxidative stress (OS) with high metabolic, energy and oxygen demands during normal pregnancy or in pregnancy-associated disorders has been reported. Taking the antioxidant properties of Trp and its metabolites into consideration, we made four hypotheses. First, the use of Trp and its metabolites is optional based on their antioxidant properties during pregnancy. Second, dynamic Trp metabolism is an accommodation mechanism in response to OS. Third, regulation of Trp metabolism could be used to control/attenuate OS according to variations in Trp metabolism during pregnancy. Fourth, OS-mediated injury could be alleviated by regulation of Trp metabolism in pregnancy-associated disorders. Future studies in normal/abnormal pregnancies and in associated disorders should include measurements of free Trp, total Trp, Trp metabolites, and activities of Trp-degrading enzymes in plasma. Abnormal pregnancies and some associated disorders may be associated with disordered Trp metabolism related to OS. Mounting evidence suggests that the investigation of the use of Trp and its metabolites in pregnancy will be meanful.

Regulatory T (Treg) cells have been identified as key immune regulators and are important to fetal survival within the maternal uterus. T helper 17 (Th17) cells have also emerged as a new subset of effector helper T cells and play significant roles in host defense against extracellular pathogens, autoimmune conditions, and inflammatory chronic diseases. Recent findings have provided strong support for the contribution of Th17 and Treg cells to successful pregnancy. Disorders of pregnancy such as preeclampsia (PE) and recurrent spontaneous abortion (RSA) are associated with low frequencies of Treg cells and high levels of Th17 cells. Here, we review current knowledge on Tregs and Th17 cells and shed light on their roles in both normal pregnancy and PE. We also discuss the imbalance between Th17 cells and Treg cells which is known as a major contributory factor in the pathophysiology of PE.
