Double purse-string around the inner cuff of the peritoneal catheter: A novel technique for an immediate initiation of continuous peritoneal dialysis

Abstract: International guidelines recommended a delayed start of peritoneal dialysis at least 2 weeks between catheter insertion and continuous peritoneal dialysis therapy initiation (break-in period). Up to now, the optimal duration of the break-in period is still unclear. The aim of our study was to evaluate in patients, with immediate initiation of continuous peritoneal dialysis, the efficacy of a double purse-string around the inner cuff in preventing mechanical and infectious complications either in semi-surgical … Show more

“…Our overall peritonitis rate was far below the threshold of 0.4 episode/patient-year proposed by the ISPD recommendations 8 . This overall good rate might be associated with the early start of in-hospital PD training (mainly the day after the catheter insertion) that patients received in our center 9,12 .…”

“…In every patient a straight double-cuffed Tenckhoff catheter was placed with a modi ed double purse string technique around the inner-cuff either in semi-surgical or surgical procedure, as described elsewhere 9,10 . The creation of the subcutaneous part was achieved by using a piercing tunneller in a direction able to minimize shear forces with the super cial cuff located at least 4 cm from the exit of the skin 11 .…”

“…The high incidence of technique failure has been recognized as a critical factor in the relatively poor PD retention rates worldwide [ 40 ]. The technique survival rate with a BI < 24 h was 95% according to a report from Korea with a 1 month follow-up period [ 20 ] and was 97.8%, according to another report from Italy with a 3 month follow-up period [ 21 ]. “…After obtaining informed consent for catheter insertion, double-cuffed Tenckhoff catheter was placed by an interventional nephrologist according to semi-surgical or surgical procedure described thoroughly elsewhere. [22][23][24] Incremental CAPD was defined as the start of PD with less than four exchanges per day, 7 days a week and a peritoneal weekly Kt/V < 1.7 per week, but a total weekly Kt/V ! 1.7 per week.…”
