Double-orifice mitral valve associated with bicuspid aortic valve: a rare case of incomplete form of Shone's complex

Abstract: Double-orifice mitral valve (DOMV) is a rare congenital malformation characterized by two separate valve orifices of varying sizes in association with the abnormalities of the subvalvular apparatus. It has been reported to be associated with a variety of other congenital malformations. We report a rare case of incomplete form of Shone's complex composed of a complete bridge-type DOMV and bicuspid aortic valve, well demonstrated by both transthoracic and transoesophageal echocardiography and cardiac magnetic re… Show more

“…13 A DOMV may be more likely to be iatrogenic from an Alfieri stitch rather than congenital if (1) there is a normal mitral subvalvular apparatus, (2) there is a medical history of surgical intervention on the mitral valve, and (3) no additional congenital heart abnormalities are present. 7,20 As with the MitraClip, the Alfieri stitch seen with 3D TEE (Fig 3, B) lacks the well-defined ridge seen in this patient. In addition, 3D TEE clearly shows the annuloplasty ring that often accompanies the Alfieri stitch, although this also can be visualized with 2D imaging.…”

“…The presence of two separate regurgitant jets on transthoracic parasternal short-axis view should arouse the suspicion of DOMV. In adults transesophageal echocardiography (TEE) is usually necessary for a definite diagnosis of DOMV [9].…”

“…The shape and outline of the thin bridge leaflet in CAVSD can be clearly displayed when TrueVue Glass is also applied. DOMV is often accompanied by multiple sets of papillary muscle malformations ( Karas et al, 2003 ; Erkol et al, 2009 ). Traditional 2D and 3D ultrasound can exactly diagnose the existence of the double orifice, but it is often difficult to display the additional structure of the mitral valve apparatus.…”
