Introduction: Cytomegalovirus (CMV) is an essential and most frequent vertically transmitted infection from mother to fetus through the placenta. Neonatal Herpes simplex virus (HSV) enters a child by physical contact with its mother’s potentially infected vaginal secretions. There are many case reports where the newborn is infected with either CMV or HSV, but only one case of a 67-year-old Chinese male with the coinfection of CMV and herpes simplex virus type II (HSV-II) 11 was found. We report a rare case of newborns with congenital infection by CMV and herpes simplex who succumb to multiorgan failure. Case Report Presentation: 8 weeks male infant came in our hospital complaining of not feeding and not gaining weight, abdominal distension, diarrhea, and jaundice. As narrated by the mother, the baby was delivered vaginally at 36 weeks of gestation, and the birth weight was 1880 grams. Mother was not recommended blood test for TORCH (toxoplasmosis, rubella cytomegalovirus, herpes simplex, and HIV, syphilis) by her obstetricians. TORCHES profile was favorable in the baby. Newborns succumb to treatment and die due to multiple organ failure. Conclusion: Torch’s panel test is used to help diagnose infections that could harm the unborn baby during pregnancy and have fatal consequences after birth; therefore, it should be mandatory in each pregnancy.

Cytomegalovirus (CMV) causes a mild illness in immunocompetent patients. Conversely, it can be life-threatening in immunocompromised or critically ill patients. We present a 48-year-old immunocompetent woman presenting primary severe CMV encephalitis. She presented with a headache, fever, and drowsiness. She did not respond to empirical treatment. Her level of consciousness deteriorated, she was put on mechanical ventilation on day two. Bacterial culture, herpes simplex virus, and tuberculosis were negative in cerebrospinal fluid (CSF). After three weeks, the patient was transferred to our center due to financial matters. Brain magnetic resonance imaging (MRI) showed diffuse hydrocephalus, periventricular T2 hyperintensity, patchy basal ganglia, and diffuse leptomeningeal enhancement. CMV polymerase chain reaction (PCR) was positive in cerebrospinal fluid (CSF) specimen. Ganciclovir (5 mg/kg/IV q12h) was initiated. Subsequently, a brain shunt was inserted. Her level of consciousness raised, she was weaned from the ventilator. She was discharged after 52 days in a bedridden state, quadriplegic, and only able to speak words with a minor swallowing problem. She remained in the same condition for one year. She was expired one year later due to aspiration pneumonia after four weeks of hospitalization. Early diagnosis and treatment of severe CMV encephalitis are crucial to prevent neurological sequelae.

Latar Belakang: Salah satu penyebab tingginya angka kematian bayi di Kabupaten Bandung adalah infeksi kongenital seperti Toxoplasma gondii, Cytomegalovirus (CMV), dan Herpes Simplex Virus (HSV). Untuk menurunkan angka tersebut dibutuhkan metode deteksi yang sensitif dan spesifik. Salah satu metode yang dapat digunakan adalah multiplex nestedPolymerase Chain Reaction (PCR).Tujuan: Mendeteksi T. gondii, CMV, dan HSV pada wanita hamil di Klinik Praktek Mandiri Bidan Anna Rohanah Kabupaten Bandung dengan metode multiplex nested PCR.Metode: Teknik pengambilan sampel yang digunakan adalah quota sampling dengan jumlah sampel sebanyak 42 orang. Gen B1 dari T. gondii, MIEA dari CMV, dan Glikoprotein D dari HSV diamplifikasi menggunakan primer spesifik. Analisis data yang digunakan adalah teknik analisis deskriptif.Hasil: Dari 42 orang wanita hamil, terdapat 3 orang (7,14%) yang terinfeksi T. gondii dan 1 orang (2,38%) yang mengalami koinfeksi CMV – HSV. Dua dari tiga wanita hamil dengan toxoplasmosis tidak menunjukkan gejala sedangkan wanita dengan koinfeksi CMV - HSV mengalami keputihan, sakit buang air kecil, gatal pada area genital, dan terdapat bintil pada mulutnya.Kesimpulan: Metode multiplex nested PCR berhasil digunakan untuk mendeteksi infeksi T. gondii pada 3 orang wanita hamil dan koinfeksi CMV – HSV pada 1 orang wanita hamil di Klinik Praktek Mandiri Bidan Anna Rohanah Kabupaten Bandung.
