Lithium augmentation is a well established strategy for treatment-resistant depression. The exact mode of its action is unknown, but an enhancement of serotonergic transmission is hypothesized. The authors investigated changes in the hypothalamic-pituitary-adrenocortical (HPA) system during lithium augmentation and their correlation to clinical response by means of the combined dexamethasone/CRH test (DEX/CRH test). Thirty patients with unipolar major depressive episodes (DSM IV) who had not responded to an adequate trial with an antidepressantwere assessed on the day before lithium augmentation (baseline) with the DEX/CRH test (pretreatment with 1.5 mg dexamethasone p.o. at 11 P . M . and CRH stimulation at 3 P . M . on the next day). Twenty-four patients were reassessed after response was determined or, in cases of nonresponse, four weeks after initiation of lithium augmentation. Response to lithium augmentation was measured by weekly ratings on the Hamilton DepressionRating Scale Regardless of the initial choice of antidepressant, about 30 to 40% of patients with major depressive disorder will not respond sufficiently (Thase and Rush 1995 Among the various treatment options that have been proposed for non-responding depressed patients, lithium augmentation has been recommended as a first-line strategy . The efficacy of lithium augmentation has been well documented in placebo-controlled acute (Joffe et al. 1993;Katona et al. 1995;Baumann et al. 1996) and continuation treatment trials (Bauer et al. 2000;Bschor et al. 2002) using different classes of antidepressants. A recent meta-analysis confirmed the evidence that lithium augmentation is superior to placebo augmentation for the treatment of unipolar major depression, with a median response rate of 50% across double-blind studies (Bauer and Döpfmer 1999). Severity of depression was recently identified as a clinical predictor of response to lithium augmentation (Bschor et al. 2001). Better knowledge of the neurobiological mechanisms of lithium augmentation would help to identify patients who will respond favorably to this treatment intervention. However, to date, the mode of action of lithium augmentation is unknown. Shortly after the initial report on its efficacy (de Montigny et al. 1981), it was postulated by de Montigny et al. (1983) that a pharmacodynamic action mediated via the serotonergic systems might account for the synergistic effect of lithium when added to a tricyclic antidepressant.Another system potentially involved in its mechanism of action is the hypothalamic-pituitary-adrenocortical (HPA) system, putatively the best studied biological system in affective disorders (Dinan 1997). Препарати літію — психотропні лікарські засоби з групи нормотиміків. Їх найважливіше значення полягає в лікуванні афективних розладів, перш за все маніакальних фаз біполярного розладу, а також у профілактиці їх загострень і лікуванні тяжких і резистентних депресій, вони здатні запобігати самогубствам. В основі лікування біполярних розладів препаратами літію лежить їх властивість селективно інгібувати кіназу-3-глікогенсинтетазу шляхом конкуренції з іонами магнію. Літій — лужний метал, тому в медицині він застосовується у вигляді солей, в основному у вигляді карбонату, а також цитрату, сукцинату, оротату, хлориду й сульфату літію. Окрім галузі психіатрії, літій та його сполуки застосовуються при виробництві пластмас, органічному синтезі, у склодувній промисловості, фармацевтичній хімії. Отруєння літієм у психіатричній практиці часто має ятрогенну природу. Літій впливає на вивільнення серотоніну й чутливість рецепторів, а також модулює дію норадреналіну. На сьогодні вважається, що механізм дії літію пов’язаний з виснаженням запасів інозитолу в центральній нервовій системі (ЦНС). Неврологічний симптомокомплекс отруєння літієм складається з атаксії, погіршення зору, втрати пам’яті, запаморочення, втрати орієнтації, судом, ступору й коми. Чим довше зберігається токсичний рівень літію в крові, тим більша ймовірність необоротних змін ЦНС. При інтоксикації літієм хворий повинен бути госпіталізований. Перша допомога: всередину натрію хлорид і розчин калію хлориду. Необхідно кілька разів на день вимірювати концентрацію літію в плазмі крові. Для швидкого зниження концентрації літію часто ефективне введення фізіологічного розчину внутрішньовенно зі швидкістю 150–200 мл на годину. Перш за все проводять промивання шлунка й кишечника (особливо при отруєнні препаратами тривалої дії). Оскільки в шлунковому соку часто виявляється висока концентрація літію, показана тривала аспірація шлунка. При епілептичних нападах, пригніченні ЦНС, артеріальній гіпотонії й аритміях лікування патогенетичне. Найбільш ефективним методом є гемодіаліз. Проте потрібно відзначити, що об’єктивно позитивні зміни при літієвій терапії (якщо вона успішна) набагато перевершують негативний вплив можливих побічних ефектів.

Background: Lithium augmentation is a first-line strategy for depressed patients resistant to antidepressive therapy, but little is known about patients’ subsequent long-term course or outcome predictors. We investigated long-term outcomes of unipolar depressed patients who had participated in a study on the effects of lithium augmentation on the hypothalamic-pituitary-adrenocortical system using the combined dexamethasone/corticotrophin-releasing hormone (DEX/CRH) test. Methods: Twelve to 28 months (mean 18.6 ± 4.6 months) after lithium augmentation, 23 patients were assessed with a standardized interview, of which 18 patients had complete DEX/CRH test results. Relapse was diagnosed by DSM-IV criteria (Structured Clinical Interview for DSM-IV; SCID I). Results: Only 11 patients (48%) had a favorable follow-up, defined as absence of major depressive episodes during the observation period. Patients with a favorable and an unfavorable course did not differ in clinical or sociodemographic parameters, endocrinological results or continuation of lithium. However, fewer previous depressive episodes tended to correlate (p = 0.09) with a favorable course. Conclusion: Results from studies using the DEX/CRH test to predict relapse in depressed patients treated with antidepressants were not replicated for lithium augmentation. Our finding could reflect the elevation of DEX/CRH results by lithium, independent of clinical course. Limitations of the study are its small sample size, the heterogeneous clinical baseline conditions and the lack of lithium serum levels. The fact that lithium continuation did not predict the course might be related to the difference between the efficacy of lithium in controlled studies and its effectiveness in naturalistic settings.
