Dosimetry advantages of intraoperatively built custom-linked seeds compared with loose seeds in permanent prostate brachytherapy

Abstract: PurposeThe aim of this study was to compare the implant quality between intraoperatively built custom-linked seeds (IBCL) and loose seeds (LS) retrospectively.Material and methodsThis study included 74 prostate cancer patients who were treated with permanent prostate brachytherapy (PPB) using IBCL (n = 37) or LS (n = 37) between July 2014 and June 2016. Dose-volume histogram (DVH) parameters, seed migration, and operation time were compared between the IBCL and LS groups. In addition to the standard target vol… Show more

“…To date, many studies compared stranded seeds to loose seeds [ 9 , 13 , 14 , 19 , 20 , 21 ], but few studies have compared the implant quality of linked seeds with that of loose seeds for prostate brachytherapy [ 17 , 22 , 23 , 24 , 25 ]; however, until now, there have been no reports about the plan reproducibility of linked seeds. Generally, intraoperative dosimetry based on ultrasound would not predict biochemical outcome [ 26 ].…”

“…Several papers have reported no significant difference in PV 100 and PD 90 between the two groups [5,8,10]. Inada et al reported that PV 100 was significantly higher in the IBCL group than in the LS group, whereas other studies showed that PD 90 was significantly lower in the IBCL group than in the LS group [7,11,12,13]. In several studies, PV 150 and UD 30 were significantly lower and HI was significantly higher in the IBCL group than in the LS group, consistent with the results of the present study [7,11].…”

“…LDR brachytherapy was first adopted in 2003 in Japan, but stranded or linked seeds were not introduced until 2012, and only loose seeds (LS) had been used for LDR brachytherapy until then [ 6 ]. Several studies have reported the advantages and disadvantages of IBCL seeds compared with LS [ 5 , 7 , 8 , 9 , 10 , 11 , 12 , 13 ]. IBCL seeds have the advantages of less migration, and stability due to intraoperative linking, indicating the benefits of loose and stranded seeds [ 14 ].…”

“…LDR brachytherapy was performed with iodine-125 seeds as strands (S06 EZAG; 6711, Oncura) rather than loose seeds [ 18 ]. A transperineal procedure monitored by transrectal ultrasound was used, under continuous control of the z-axis by fluoroscopy until December 2005, exclusively ultrasound-based as real-time planning after December 2005.…”
