“Dose of the day” based on cone beam computed tomography and deformable image registration for lung cancer radiotherapy

Abstract: Purpose: Adaptive radiotherapy (ART) has potential to reduce toxicity and facilitate safe dose escalation. Dose calculations with the planning CT deformed to cone beam CT (CBCT) have shown promise for estimating the "dose of the day". The purpose of this study is to investigate the "dose of the day" calculation accuracy based on CBCT and deformable image registration (DIR) for lung cancer radiotherapy. Methods: A total of 12 lung cancer patients were identified, for which daily CBCT imaging was performed for tr… Show more

“…The spinal cord of both H&N and thoracic body site and the parotid gland showed the largest deviations amongst all OAR dose parameters for both the H&N and thoracic region, having dose difference values of up to 14.5%. Comparably large dose deviations were also obtained by a DIR-based CBCT correction approach with − 5.55% ± 14.3% for the thoracic spinal cord [48]. For the H&N body site, absolute mean dose differences for the parotid gland and the spinal cord were reported as 0.05-0.07 Gy and 0.42 Gy [35] which compares to our results by a factor of approximately 0.5.…”

“…Improved computer methods such as deformable registration of planning CTs to create virtual CTs matching up-to-date CB-CTs ( 19 , 70 ) are also promising. Further advances in image reconstruction and procession, e.g.…”

“…Therefore, frequent adaptation of the target volume and hence the treatment plan might be applied to limit toxicity by reducing dose to adjacent critical structures. In previous studies, however, adaptation was usually performed only once or twice during the course of radiotherapy (15)(16)(17)(18)(19).…”

“…Interestingly, a recent study showed that higher tumor regression may be an adverse prognostic factor for CHRT, and a favorable prognostic factor for RT alone (38). Several studies have postulated the benefits of adaptive RT in locally advanced NSCLC (39)(40)(41). This approach is being investigated in the ongoing RTOG 1106 trial, results of which are expected shortly (42).…”
