The blood supply from the BFP comes from branches of the internal maxillary, superficial temporal, and facial arteries. This complication may be associated with deep dissection of the oral space or a vascular avulsion injury from traction on the BFP. If local measures such as compression and blood vessel suture fail to achieve hemostasis, other methods to stop bleeding should be adopted immediately to prevent further severe complications that can be life-threatening. 13,14 Careful removal of this structure should be performed with precise indication for its removal. 21 Although bichectomy is being widely performed on a clinical basis, there are not enough long-term follow-up studies that can evaluate the effect in facial aging, function, and report complications. 22 A retrospective study analyzed data regarding buccal fat pad excision highlights the importance of long-term patient follow up to access information about satisfaction level and postoperative complication. 17 A recent systematic review evaluated eight articles in order to identify the indications, complication types and rates, surgical techniques, and outcomes, all of it achieved a high risk of bias, and none of them were retrospective or prospective clinical trials. 7 CONCLUSIONComplications need to be prevented, and knowledge is required for the management and treatment of these complications.

Background An essential aspect of rhinoplasty is the enhancement of the nasal dorsal contour by performing dorsal augmentation (DA) rhinoplasty. A wide range of techniques are available for DA as the demand for aesthetic nasal refinement grows. This review aims to provide a comprehensive overview of the current techniques used in DA rhinoplasty. Main body Research articles on DA rhinoplasty techniques were identified through a comprehensive literature search. Scopus, PubMed, and Web of Science were used as electronic databases. Each database was searched for articles published since its inception. DA rhinoplasty techniques were examined in this literature review. Methodological quality was assessed for the selected studies, and data was extracted to examine materials used, surgical approaches, and reported outcomes for each technique. Various DA methods, including autologous grafts and synthetic implants, are examined in-depth in this review. Comparing approaches can help better understand their respective advantages and limitations. Conclusion A wealth of techniques is available for DA rhinoplasty, each with advantages. Patients’ nasal anatomy, desired outcomes, and potential risks must be considered by surgeons when determining their surgical approach. DA methods continue to evolve rapidly, creating a need for a thorough understanding of the current landscape to make informed decisions.

Effect of Stromal Vascular Fractions on Angiogenesis of Injected Diced Cartilage

Objectives: This study explored the effect of adipose-derived stromal vascular fractions (SVFs) on angiogenesis in injected autologous diced cartilage. Methods: Stromal vascular fractions were extracted by enzymatic digestion. Cartilage grafts were harvested from 1 side of the auricular cartilage of New Zealand rabbit and then diced to a size of 1.0 mm 3 . The grafts were divided into 2 groups. The control group was diced cartilage mixed with culture medium, and the experimental group was diced cartilage mixed with SVFs. The 2 groups of composite grafts were subcutaneously implanted on both sides of the back of each rabbit. After 4, 12 and 24 weeks, the tissue structure, number of blood vessels, and angiogenic factors in the grafts were observed. Results: The SVFs conformed to the current standard of the biological evaluation. Under an inverted microscope, the number of layers of chondrocytes in the experimental group was higher than that in the control group at 4 weeks. A small number of inflammatory cells and blood vessels were observed around the cartilage grafts. At 12 and 24 weeks, the volume of tissue was increased gradually by general observation. And a large number of chondrocytes were observed microscopically, whereas the number of inflammatory cells decreased. And meanwhile additional new blood vessels were observed. Immunohistochemical analysis of CD31 showed that the number of capillaries in the control group was significantly lower than that in the experimental group at 4, 12 and 24 weeks. Further, the expression of Hypoxia inducible factor-1 (HIF-1) and vascular endothelial growth factor (VEGF) mRNA and protein were measured by RT-PCR and Western bloting, respectively. The results showed that the mRNA expression of VEGF and HIF-1a in the experimental group was increased. The mRNA level remained higher than that of the control group at 24 weeks (P < 0.05). And the relative expression levels of VEGF and HIF-1a protein in the experimental group were higher than those in the control group at 4, 12 and 24 weeks (P < 0.05). Conclusion: Autologous diced cartilage mixed with adiposederived SVFs can promote angiogenesis when transplanted by injection. Further research showed that SVFs could increase the expression levels of VEGF and HIF-1a in the grafts, which may be part of the mechanism that SVFs promoted the angiogenesis of diced cartilage.
