Au cours des dernieres ann~es, les nombreuses donnees fondamentales ~lucidant les mecanismes Iocaux de I'~rection ont permis de proposer des traitements des dysfonctions ~rectiles par des m~dicaments a action p~ripherique. Ces produits permettent soit de provoquer une erection, ils sont alors efficaces seuls, soit de potentialiser une ~rec-tion.Dans un environnement sexuellement stimulant, I'~rection est provoquee par une modification de I'equilibre des effets antierectiles du syst~me sympathique et des effets pro~rectiles du syst~me parasympathique. Les voies sympathiques et parasympathiques naissent de la moelle ~pini~re. Le syst~-me nerveux central agit sur la moelle (~pini~re pour contr61er et modifier cet equilibre sympathiqueparasympathique. La dopamine est un neurom~dia-teur du syst~me nerveux central agissant sur le comportement sexuel et sur les noyaux centaux contr61ant le tractus g~nital chez le m&le. Les taux de dopamine augmentent dans plusieurs noyaux centraux (amygdale, aire preoptique mediane) pendant I'activit~ sexuelle chez le rat. Les agonistes dopaminergiques a action centrale comme I'apomorphine (agoniste DI/D2) provoquent I'~rection chez I'homme, le singe et le rat. Chez le rat, les effets proerectiles de I'apomorphine sont dus ~ sa fixation sur des recepteurs de type D2 dans le noyau paraventriculaire de I'hypothalamus (PVN), et seraient regul(~s par la testosterone. Des investigations cliniques r~centes ont mis en avant les avantages de I'utilisation de I'apomorphine pour traiter les patients souffrant de dysfonctions ~rec-tiles. Une molecule a action centrale comme I'apomorphine pourrait r~organiser I'~quilibre sympathique-parasympathique, conduisant & un recrutement adapte des voies nerveuses vers le p~nis. Neurobiology of Sexual Desire

The aim of this paper is to elucidate the anatomical and molecular nature of sexual desire. As such we have focused our attention to the telodiencephalic reproductive complex and the functional interactions with the cortico-limbic circuit that regulate sexual and non-sexual motivation. Major focus of our review was on the animal studies that included hormones, peptides, neurotransmitters and the unique study paradigms that were designed to separate sexual motivation from the consumatory behavior. We also have covered limited number of clinical trials but our primary goal was to review the animal study results. We present rapidly evolving animal research data that we hope will contribute toward the development of new drugs that ameliorate the symptoms of hypoactive sexual desire disorders.

The use of the D 1 =D 2 dopamine receptor agonist apomorphine SL for the treatment of erectile dysfunction provides a strong support in favour of a participation of the dopaminergic system in the control of sexual function. However, the exact involvement of dopamine in sexual motivation and in the control of genital arousal in humans is unknown. In contrast, experimental data suggest an implication of dopamine at all these stages of the copulatory behaviour in rodents. The release of dopamine at the level of the nucleus accumbens, which is innervated by the mesolimbic dopaminergic pathway originating in the ventral tegmental area, is positively implicated in the pre-copulatory or appetitive phase in male rats. There is also a permissive role in the copulatory or consumatory phase for dopamine released at the level of the median pre-optic area, which receives projection from the dopaminergic incertohypothalamic pathway within the hypothalamus. It is noteworthy that these participations of the dopaminergic system are not specific to sexual behaviour but rather reflect the more general involvement of dopamine in the regulation of cognitive, integrative and reward processes. Due to its role in the control of locomotor activity, the integrity of the nigrostriatal dopaminergic pathway is also essential for the display of copulatory behaviour. Somehow more specific to sexual function, it is likely that dopamine can trigger penile erection by acting on oxytocinergic neurons located in the paraventricular nucleus of the hypothalamus, and perhaps on the pro-erectile sacral parasympathetic nucleus within the spinal cord. The counterpart of such regulation of the genital arousal by dopamine has not yet been established in females. In conclusion, the central dopaminergic system is a key element of the control of sexual function.
