Association between the Use of Pessary and the Length Use of Pessary with the Incident of Bacterial Vaginosis on Pelvic Organ Prolapse Patients

Introduction and Hypothesis: Pelvic Organ Prolapse (POP) is a condition with specific signs such as descent of the fornix anterior, fornix posterior, uterine, cervix, apex of the vagina after hysterectomy or perineal. The use of pessary is a conservative and the first line of POP medication. Most women who used pessaries complained about vaginal discharge, more than 50% of them were proven suffered from bacterial vaginosis. This research was aimed to know whether there was any association between the incident of BV with the use of pessaries and the length use of pessaries among POP patients. We hypothesized that the use of pessaries and the length use of pessaries had a significant association with the incident of BV. This study was a case-control study. There were 68 medical records, which were obtained through purposive sampling. Independent variables were the use of pessaries and the length use of pessaries, the dependent variable was the incident of BV among POP patients. Data collection sheets were the instrument of this research. Chi-Square, Fisher Exact Test and Odd ratio were used to analyze the data. This research used α: 5%, CI: 95% Result: This research revealed that there was a significant association between the incident of BV on POP patients with the using of pessaries (P=0.001) and the length use of pessaries (P=0.024). Odd ratio 5.9 (95% CI, 1.95-17.97) means patients with POP who used pessaries had a probability to suffer BV 5.9 times higher than those who didn't. Conclusion: there was a significant association between the incident of BV with the use of pessaries and the length use of pessaries on POP patients.

Penelitian ini bertujuan mengetahui perbedaan kekuatan otot levator ani dan otot menggenggam sebelum dan sesudah pemberian analog vitamin D3 (alfacalcidol) dan mengetahui korelasi antara kekuatan kontraksi otot levator ani dan kekuatan kontraksi otot menggenggam pada penderita prolapsus organ panggul. Metode: Penelitian ini merupakan penelitian eksperimental quasi dengan rancangan sebelum dan sesudah pemberian alfacalcidol pada pasien prolapsus organ panggul. Dilakukan pengukuran kekuatan otot levator ani menggunakan perineometer dan otot menggenggam menggunakan handgrip dynamometer sebelum dan sesudah pemberian alfacalcidol selama 3 bulan. Penelitian dilakukan di Poliklinik Ginekologi FKUP/RSHS bulan Januari-Juli 2021. Hasil: Dilakukan pemberian suplementasi alfacalcidol selama 3 bulan pada 24 penderita prolapsus organ panggul. Hasil evaluasi menunjukkan adanya perbedaan yang bermakna secara statistik (nilai p <0,001) pada sebelum dan sesudah pemberian suplementasi alfacalcidol yaitu terjadi peningkatan kekuatan otot levator ani (rerata pre= 9,94±3,56 dan pasca= 19,67±3,61) dan terdapat peningkatan kekuatan otot menggenggam (rerata pre= 15,39±3,29 dan pasca= 23,90±3,67). Hasil penelitian ini menemukan adanya korelasi antara peningkatan kekuatan kontraksi otot levator ani dan kekuatan kontraksi otot menggenggam (nilai p<0,01) yang memiliki keeratan hubungan yang moderat (nilai R pre= 0,532 dan pasca= 0,618). Simpulan: Suplementasi alfacalcidol dapat meningkatkan kekuatan kontraksi otot levator ani dan otot menggenggam pada penderita prolaps organ panggul. Terdapat korelasi yang bermakna dengan hubungan moderat pada kekuatan kontraksi otot levator ani dengan kekuatan kontraksi otot menggenggam pada penderita prolapsus organ panggul.

Background: Vaginal delivery is a risk factor for pelvic organ prolapse. The biomolecular mechanism that links it is still unclear, thought to be through Matrix Metalloproteinase 9 (MMP-9). Purpose: This study aims to determine the increase in MMP-9 in the supporting tissues of the pelvic organs, especially the sacrouterine ligament and levator ani muscles in the postpartum period. Methods: This study was an observational cohort study on 5 female rats of Rattus norvegicus Wistar strain. Which were divided into nulliparous groups, postpartum day 1, day 3, and day 7. MMP-9 expression was calculated and analyzed using ANOVA. Results: There was a significant difference (p=0.01) of MMP-9 expression in the sacrouterine ligament between nulliparous rats, postpartum day 1, day 3, and day 7. There was also a significant difference (p=0.00) levator ani muscle MMP-9 expression between nulliparous rats, postpartum day 1, day 3, and day 7. There was a trend of increasing MMP-9 expression on the 1st to 3rd day postpartum, then decreasing gradually on the 7th day. Conclusion: Delivery changes the expression of MMP-9 in pelvic floor tissues such as the sacrouterine ligament and levator ani muscle and these changes are time-dependent.Abstrak. Latar Belakang: Persalinan pervaginam merupakan faktor resiko prolaps organ panggul. Mekanisme biomolekuler yang menghubungkannya masih belum jelas, diduga melalui perantaraan Matriks Metalloproteinase 9 (MMP-9). Tujuan: Penelitian ini bertujuan untuk mengetahui peningkatan MMP-9 pada jaringan penyokong organ panggul, terutama ligamentum sacrouterina dan otot levator ani periode post partum. Metode: Penelitian ini adalah studi observational cohort pada 5 tikus betina Rattus norvegicus strain Wistar. Yang terbagi menjadi kelompok nullipara, postpartum hari ke-1, hari ke-3 dan hari ke-7.. Preparat jaringan ligamentum sacrouterina dan otot levator ani diwarnai dengan imunohistokimia. Ekspresi MMP-9 dihitung dan dianalisa dengan menggunakan ANOVA. Hasil: Terdapat perbedaan yang signifikan (p=0,01) ekspresi MMP-9 pada ligamentum sacrouterina antara tikus nullipara, postpartum hari ke-1, hari ke-3, dan hari ke-7. Terdapat pula perbedaan yang signifikan (p=0,00) ekspresi MMP-9 otot levator ani antara tikus nullipara , postpartum hari ke-1, hari ke-3, dan hari ke-7. Tampak tren peningkatan ekspresi MMP-9 pada hari ke-1 hingga ke-3 postpartum, selanjutnya menurun bertahap pada hari ke-7. Simpulan: Persalinan mengubah ekspresi MMP-9 pada jaringan dasar panggul seperti ligamentum sacrouterina dan otot levator ani dan perubahan ini bersifat time-dependently.
