Background: Rectal cancer is a common malignancy. Since the introduction of bowel-screening programs, the number of patients with advanced adenomas and early rectal cancer has increased. Despite improved diagnostics, the discrimination between rectal adenomas and early rectal cancer (i.e., pT1–T2) remains challenging. The purpose of this systematic review was to evaluate the diagnostic performance of endorectal ultrasound (ERUS) elastography in discriminating rectal adenomas from cancer. Method: Using PRISMA guidelines, a systematic search was performed on PubMed, Embase, and MEDLINE databases. Studies evaluating the primary staging of rectal adenomas and cancer using ERUS elastography were included. Results: Six studies were identified; three evaluated the discrimination between adenomas and cancer; two evaluated adenomas and early rectal cancer (i.e., pT1–T2); one evaluated performance on different T categories. All studies reported increased diagnostic accuracy of ERUS elastography compared to ERUS. Sensitivity, specificity and accuracy ranged 0.93–1.00, 0.83–1.00 and 0.91–1.00, respectively, when discriminating adenomas from cancer. In the differentiation between adenomas and early rectal cancer, the sensitivity, specificity and accuracy were 0.82–1.00, 0.86–1.00 and 0.84–1.00, respectively. Conclusion: Elastography increases the accuracy of ERUS and may provide valuable information on malignant transformation of rectal lesions.

Background Ultrasound (US) elastography has become a routine instrument in ultrasonographic diagnosis that measures the consistency and stiffness of tissues. Purpose To distinguish benign and malignant breast masses using a single US system by comparing the diagnostic parameters of three kinds of breast elastography simultaneously added to B-mode ultrasonography. Material and Methods A total of 163 breast lesions in 159 consecutive women who underwent US-guided core needle biopsy were included in this prospective study. Before the biopsy, the lesions were examined with B-mode ultrasonography and strain (SE), shear wave (SWE), and point shear wave (STQ) elastography. The strain ratio was computed and the Tsukuba score determined. The mean elasticity values using SWE and STQ were computed and converted to Young’s modulus E (kPa). Results All SE, SWE, and STQ parameters showed similar diagnostic performance. The SE score, SE ratio, SWEmean, SWEmax, STQmean, and STQmax yielded higher specificity than B-mode US alone to differentiate benign and malignant masses. The sensitivity of B-mode US, SWE, and STQ was slightly higher than that of the SE score and SE ratio. The SE score, SE ratio, SWEmean, SWEmax, STQmean, and STQmax had significantly higher positive predictive value and diagnostic accuracy than B-mode US alone. The area under the curve for each of these elastography methods in differentiating benign and malignant breast lesions was 0.93, 0.93, 0.98, 0.97, 0.98, and 0.96, respectively; P<0.001 for all measurements. Conclusion SE (ratio and score), SWE, and STQ had higher diagnostic performance individually than B-mode US alone in distinguishing between malignant and benign breast masses.

Background To investigate the feasibility of combination of ultrasonic elastography (UE) and Ki-67 index as a novel predictive modality for the prognosis of clinic stage Ⅲ breast cancer. Methods Of 112 patients, Multi-parameter of UE were obtained after analyzing elastograms of 112 lesions which were recorded classification of Ki-67 by histopathologic diagnosis. Multivariate Survival analysis was performed by using multivariate Cox model. The nomograms was established to predict the probability of clinic stage Ⅲ breast cancer according to the selected independently significant variables in the multivariate Cox model. The model was internally validated using Harrell’s concordance index. Results ⅢC had significant difference in all ultrasonic elastography (UE) parameters (P < 0.05) and were statistically different from ⅢA and ⅢB (P < 0.01). The Cox model was RR (t)= Exp(1.104* X1+0.901*X4+0.972*X5) (X1: classification of Ki-67, X4: classification of strain ratio, X5: classification of Young's modulus). The nomogram showed that Ki-67 had the greatest influence on survival prediction, followed by Young's modulus and strain ratio. Internal validation revealed a concordance index of 0.76. Conclusions Our study explored a novel modality in predicting the prognosis of clinic stage Ⅲ breast cancer combining a tumor marker with breast UE imaging. A nomogram was developed to prove that the model was helpful to estimate the 5-year survival of the patients.
