Does recipient work status pre-transplant affect post–heart transplant survival? A United Network for Organ Sharing database review

Abstract: This study is the first analysis of UNOS STAR data on recipient work status pre-HTx demonstrating: (1) an improvement in post-transplant survival for working HTx candidates; and (2) an association between working pre-HTx and longer post-HTx survival. Given that work status before HTx may be a modifiable risk factor for better outcomes after HTx, we strongly recommend that UNOS consider these important findings for moving forward this patient-centered research on work status. Working at listing and working at H… Show more

“…Heart recipients also are more likely to work after receiving the graft if they have been previously employed [8]. Moreover, Ravi et al observed that the patients who had worked prior to heart transplantation presented lower 5-year and 10-year mortality rates after the procedure [14]. Salaried work does not only provide the necessary means for survival but also improves the quality of life.…”

“…The registry tracks every organ donation and transplant occurring in the United States and publishes pre‐transplant and post‐transplant data on an online database 25 . Data was obtained from the “Standard Transplant Analysis and Research” (STAR) files, “Thoracic” section on heart transplant patients, as previously described 26 . The public‐use database study was deemed exempt from institutional review board approval.…”

“…According to the employment questionnaire for liver transplant patients (Wang et al, 2013), employment states were divided into five responses: very poor, poor, fair, good and very good and the 5‐point Likert scale was used to score them as 1, 2, 3, 4 and 5, respectively. Studies have shown that a positive employment state is associated with increased work satisfaction and engagement, and a better state to cope with stress (Ravi et al, 2018; van Hooff & Geurts, 2015), thereby can maintain the sustainability of employment and improve the quality of employment. Because the employment state is linked to a variety of positive outcomes, it is necessary to investigate factors that could influence it.…”
