Does Motion Assessment With 4-Dimensional Computed Tomographic Imaging for Non–Small Cell Lung Cancer Radiotherapy Improve Target Volume Coverage?

Abstract: Introduction:Modern radiotherapy with 4-dimensional computed tomographic (4D-CT) image acquisition for non–small cell lung cancer (NSCLC) captures respiratory-mediated tumor motion to provide more accurate target delineation. This study compares conventional 3-dimensional (3D) conformal radiotherapy (3DCRT) plans generated with standard helical free-breathing CT (FBCT) with plans generated on 4D-CT contoured volumes to determine whether target volume coverage is affected.Materials and methods:Fifteen patients … Show more

“…Different retrospective studies have previously evaluated the significance of 4D CT simulation in radical lung cancer radiotherapy treatment. Ahmed et al made an evaluation to find out if the motion assessment with 4D CT simulation improves the target coverage in lung cancer radiotherapy and showed superior coverage of the target volume compared to 3D simulation (20). iGTV 4D was significantly larger than GTV 3D for both primary and nodal disease, either combined or separately.…”

“…S obzirom da tretmanske tehnike kao što su 2DRTi 3DCRT nisu mogle ispuniti ove parametre, tehnološke inovativne tehnike kao što je Intenzitetom modulisana zračna terapija uz 4DCT simulaciju su uvedene u tretman karcinoma pluća, kako bi uspjeli zadovoljiti svi ovi izazovi 12 .…”

“…For treatment planning, treatment verification, and adaptive radiation in many malignancies, accurate Computed Tomography (CT) studies in the presence of respiratory motion are required [1]. The use of radiotherapy for lung cancer is growing, with Stereotactic Ablative Radiotherapy (SABR) currently being recommended as the first-line therapy for patients with Stage I NSCLC who are medically inoperable and require non-invasive treatment options [2]. To achieve the maximum therapeutic ratio, the underlying concept of administering a high and preferentially tumoricidal dosage to a target location while administering a low dose to nearby essential structures must still be followed [3].…”
