Does Intravesical Chemotherapy Prevent Invasive Bladder Cancer?

Abstract: Intravesical chemotherapy has been shown to prolong the interval free of disease and to reduce the tumor recurrence rates in patients with superficial bladder cancer. These observations led us to consider whether a course of intravesical chemotherapy might provide a long-term decrease in the recurrent tumor rate or reduce the incidence of progression to invasive carcinoma. The records of 123 patients entered into a randomized multicenter protocol between 1975 and 1978 were examined. Patients had received a 1-y… Show more

“…Earlier reports claiming success in progression-free survival with adjuvant chemotherapy were not confirmed. 15 The pathology review revealed considerable changes in pT category from stage pT1 to pTa disease. Muscle invasive tumors were missed in 4 of the 318 cases reviewed (1.25%).…”

“…The occurrence of neoplasms in the urinary tract epithelium is known to relate to multistage and multifocal carcinogenesis. A high recurrence rate of superficial bladder tumors, even after curative transurethral resection, has often been reported (4). These successive, recurrent tumors may increase in their histological grade, and more than 15% of the patients are predicted to suffer the progression with subsequent poor prognosis (5,6).…”

“…Randomized trials have shown topical chemotherapy to be ineffective in preventing tumor progression in superficial bladder carcinoma [24][25][26][27][28][29]. Metaanalysis of EORTC and MRC randomized trials proofed the impact of intravesical chemotherapy on tumor progression, metastases, survival and tumor-related death in 2,535 patients.…”
